Restorative Aide (*Must be a CNA)

Job Summary:

Reports To: Restorative Nurse/Charge Nurse

Assume the responsibility for specific tasks as directed by the Restorative/Charge Nurse.

The Restorative Nursing Assistant is a Certified Nursing Assistant that has been given additional

training in the area of rehabilitation, which provides restorative care to the assigned residents.

Must be proficient in motivating residents in attending and participating in their restorative

program. Must be able to communicate clearly, read, and write.

Responsibilities Include:

ESSENTIAL REQUIREMENTS, DUTIES, AND RESPONSIBILITIES

1. Transport residents to and from appointments in house when applicable

2. Coordinates Restorative Programs with other departments as needed to ensure

Program is not missed.

3. Able to clearly follow the written word, document resident outcomes on a daily basis,

and communicate changes observed or voiced to immediate supervisor

4. Participates in implementing and gives an opinion on the development of the individual

resident restorative program

5. Provides specific Restorative program(s) as per residents’ plan of care after receiving

additional training in aspects of restorative Nursing; reports changes noted to the

charge nurse, nursing supervisor, ADON in a timely manner.

6. Participates in the Restorative meetings

7. Works collaboratively with the Therapy Department to ensure appropriate restorative

activities

8. Ensures that all adaptive equipment necessary for the individual restorative program is

readily available, clean and in good working order

9. Completes all required restorative documentation

10. May complete the Quarterly Restorative note which will be reviewed and signed off

by the License Nurse

11. Answer call bells and alarms promptly.

12. Obtain and record vital signs as needed.

13. Report to charge nurse and/or unit manager, supervisor, DON any changes in

condition and/or behavior of resident.

14. Attend unit conferences and in-service programs as assigned.

15. Comply with facility policies and procedures including fire prevention and safety

standards.

16. Employment is contingent on successful completion of the pre-placement physical

and criminal background check.

17. Is subject to exposure of infectious waste, diseases, condition, etc, including the

AIDS and Hepatitis B viruses.

18. Must complete the State approved training and competency evaluation program for

nurse aide. Must be competent to provide nursing and nursing related services as

demonstrated in the nurse aide registry.

19. Regular attendance required.

20. Other duties as assigned by supervisor.

ESSENTIAL PHYSICAL AND MENTAL JOB REQUIREMENTS

Must be proficient in reading and writing and understand the spoken and written word. Must be

able to apply good sound nursing judgement while applying the Nurse Practice Act. Must have

some computer skills. Must be able to hear with corrective or adaptive devices and see adequately with corrective eyewear. Must have simple computational skills such as addition, subtraction, multiplication, division, and percentages. Must be able to stand for long periods of time, bend at knees and waist, push and pull, turn and lift a minimum of 20lbs

Schedule:

Monday - Friday 8 am - 4 pm Every other weekend work is required
