Shift Information: The position is PRN working as needed by the department.

What you will do: The Social Worker, in collaboration with the clinical team, provides patient and family advocacy, discharge planning, and psychosocial intervention. The Social Worker strives to promote patient and family wellness, improved care outcomes, and access to appropriate hospital and community resources. When indicated, the Social Worker will assist with crisis intervention and legal reporting requirements.
Required Education: Graduation from an accredited School of Social Work; Master's degree in Social worker; current LMSW
Preferred Education: CCM Certified
Required Skills: Basic computer and keyboarding.
Preferred Skills: Meditech Electronic Medical Record.
Required Experience: Three years social work experience.
Preferred Experience: Experience in the hospital setting.
