This position offers the opportunity to provide compassionate, comprehensive care to skilled nursing residents while working collaboratively with physicians, nursing staff, and facility leadership. Position Highlights:Setting: Skilled Nursing Facility (SNF) / Post-Acute Care Schedule: Mondayâ€"Friday, daytime rounding with shared daytime call rotation (including weekends) Responsibilities Include: - Provide primary medical management to residents - Perform initial and follow-up assessments, develop treatment plans, and manage chronic and acute conditions - Collaborate with attending physicians, nursing staff, and interdisciplinary teams - Promote continuity of care and facilitate transitions between care settings - Participate in quality improvement and compliance initiatives Candidate Qualifications:Certified and licensed Nurse Practitioner (NP) in the state of Pennsylvania Background in Family Medicine, Internal Medicine, or Emergency Medicine Previous experience in post-acute or geriatric care preferred but not required Strong interpersonal and communication skills with a focus on patient-centered care Compensation & Benefits:Estimated Annual Compensation: $117,100 â€" $138,520 - No cap on productivity income potential Employment Type: W2 Employee Comprehensive Benefits Package including: - Health, Dental, and Vision Insurance - Retirement Savings Options - Paid
