Outreach & Program Implementation Coordinator (Part‑Time Contract) Contract Duration:

Part‑time contract position over 18 months, funded through Community Capacity Building Funds grant.

Hours:

Approximately 20 hours/week (flexible schedule, some evenings/weekends required)

Compensation:

DOE $35–40/hour

Location:

Lincoln County, Oregon (hybrid: in‑person and remote work)

Grow Lincoln County is seeking a passionate, culturally responsive, and community‑centered

Outreach & Program Implementation Coordinator

to help us advance our health‑related social needs (HRSN) initiatives. This part‑time, contract‑based position will play a key role in implementing nutrition‑focused programming, expanding outreach and engagement efforts, and supporting families in accessing vital HRSN‑aligned services.

Key Responsibilities 1. Outreach & Engagement (40%)

Collaborate with Lincoln County School District (LCSD) and other partners to identify and reach students and families eligible for HRSN supports.

Conduct culturally and linguistically appropriate outreach to families, schools, and community members with a focus on building trusted relationships.

Coordinate and promote participation in community events, workshops, and garden‑based health education programs.

Ensure all outreach materials are accessible, culturally relevant, and available in participants’ preferred languages.

2. Program Implementation (35%)

Coordinate vouchers for food preservation and gardening programs and assist with participant enrollment in evidence‑based programs.

Help facilitate or coordinate medically tailored meals access and wraparound referrals for eligible students and families.

Coordinate with local partners to implement the Veggie Rx program and support participant navigation and outreach.

3. Systems Navigation & Service Coordination (15%)

Provide warm hand‑offs and referrals to other HRSN‑aligned services (housing, food access, behavioral health).

Navigate service systems (Unite Us) and Medicaid‑billable programs.

Maintain participant confidentiality in accordance with HIPAA standards and support secure information‑sharing with LCSD.

4. Administrative & Evaluation Support (10%)

Track participation, referrals, and outcomes for billing, grant reporting, and continuous improvement.

Contribute to the development of internal policies and procedures that support sustainable HRSN program delivery.

Provide feedback to improve service delivery based on community input and emerging needs.

Required Qualifications

Experience in community outreach, health education, or program coordination.

Excellent interpersonal, communication, and organizational skills.

Strong cultural humility and experience working with Latinx, Indigenous, and immigrant communities.

Ability to work flexible hours and travel within the county as needed.

SSN or ITIN or willingness to apply for one.

Preferred Qualifications

Bilingual in English and Spanish strongly preferred.

Familiarity with Lincoln County’s community landscape and health/social service resources.

Experience with garden‑based, nutrition, or food systems programming.

Knowledge of HRSNs, Medicaid billing pathways, and/or Unite Us platform.

Background in community health work, social services, or public health.

Experience coordinating or facilitating workshops or educational programming.
