Job Summary

The Certified Diabetes Educator II helps people at risk and with diabetes. They aim for behavior changes that lead to better clinical outcomes and improved health.

100% onsite position

Two main clinic locations: Bee Cave Clinic and Austin River Place. Other satellite locations may include Buda, Hutto, Taylor, Cedar Park, Pflugerville (schedule TBD).

Essential Functions Of The Role

Plans, directs, and provides personalized self‑management education, training, and support to patients and families in a culturally competent way throughout their lives.

Develops and conducts educational programs and in‑service training programs.

Serves as a resource to staff and community as a diabetes care and education specialist.

Optimizes care delivery, patient and provider experience, quality, safety, and costs.

Applies current principles of teaching and learning and/or behavior change to facilitate self‑management skills.

Tailors interventions to personal self‑management needs.

Communicates information to patients and the healthcare team promptly. Serves as a role model, resource, and mentor to other team members.

Participates in quality improvement activities to help process improvement and clinic/facility/system goals.

Participates in professional development activities to maintain and improve required qualifications.

Serves as program coordinator and oversees diabetes education program, including managing staff, if applicable.

Key Success Factors

Advanced knowledge of pre‑diabetes and diabetes care modalities, therapies, terminology, and equipment.

Demonstrates familiarity with pre‑diabetes and diabetes pathophysiology, epidemiology, and evidence‑based clinical practice.

Demonstrates familiarity with ADA Education Recognition requirements or National Standards for Diabetes Self‑Management Education and Support in an ADA Recognized program.

Must be able to communicate thoughts clearly; both verbally and in writing.

Ability to effectively use a variety of teaching methods.

Knowledge of the geographic area, the community and its resources.

Cultural proficiency related to health practices.

Committed, dependable, adaptable, inventive, resourceful and has an exceptional desire to help the community.

Able to interact positively with patients as needed.

General computer skills include using software applications, data entry, information security, electronic medical documentation, handheld scanning, and email.

Registered Dietitian strongly preferred.

Knowledge of insulin pumps and continuous glucose monitoring (CGM) devices strongly preferred.

Qualifications

Education: Bachelor’s degree.

Experience: 2 years of experience.

Certification / License / Registration

Basic Life Support (BLS): BLS or BLS within 30 days of hire or transfer.

Cert Diabetes Educator (CDE): Must have a Certified Diabetes Educator Certification (CDE).

Licensed Dietitian (RD/LD), Registered Nurse (RN):
