JOB SUMMARY

— Manages clinical activities, priorities and problems, pharmacist staffing and workloads. Provides status report to pharmacy management effectively communicating issues, concerns, and suggestions as appropriate. Manages and develops clinical initiatives and determines therapeutic alternatives when drug shortages are identified and communicates with medical staff leadership and key stakeholders. Assists with budget preparation and daily financial activity specifically in relation to high cost drugs. Assists the Director and Assistant Vice President of Pharmacy with meeting administrative goals and objectives.

The coordinator will be primarily responsible for coordination of operations related to the Pharmacy & Therapeutics Committee and the formulary management process.

Responsibilities

Partners with clinical pharmacy managers to implement cost-reduction and formulary optimization initiatives, including drug class reviews, biosimilar conversions, MUEs, and monographs

Oversight and implementation of P&T Committee decisions (i.e., EHR updates, education, and communication)

Facilitates processes across departments to develop and execute formulary changes that mitigate the impact of drug shortages system-wide

Qualifications

Education

— Bachelor\'s Degree (Bachelor of Science) or PharmD from ACPE-accredited school of pharmacy

Experience

— 2 years of experience required or equivalent pharmacy residency experience; English (speaking, reading, writing)

Licenses and Certifications

— BLS (Basic Life Support) certification required by start date; eligible for licensure by Texas State Board of Pharmacy (TSBP) with Texas license within 30 days of hire

Experience and Education (continued)

Preferred experience

— Completion of a PGY1 & PGY2 residency from an ASHP-accredited program with 1 year of experience; PGY1 with 5 years of experience; or 8 years of experience in a progressive clinical practice setting

Previous experience in health system pharmacy leadership, with responsibility for managing direct reports

Background in project management, including planning, execution, and cross-functional coordination

Job Duties

Performs job responsibilities in accordance with organizational and departmental policies and procedures, performance improvement objectives and environmental/infection control standards. Develops/implements process to meet/comply with JCAHO and regulatory requirements.

Coordinates staff development, training, competencies, certification, and continued education. Develop, implement, and manage the clinical practice of the staff and department. Assists clinical staff pharmacists with therapeutic monitoring, patient assessments, interpretation of lab values, TPN assessment and pharmacokinetic calculations. Identify opportunities for process improvements, and develop and implement policies and procedures for prompt, safe and efficient patient care.

Actively promotes pharmacy. Coordinates and participates in multidisciplinary in-services/meetings, publishes articles (i.e. staff newsletter), individualized drug information, rounds, consults, etc. Maintains high visibility/availability through participation in in-services, meetings, and daily activities. Participates in the education/experiential training of pharmacy students.

Actively manage P&T initiatives through monitoring of documented interventions. Verifies data, develops measures and reports outcomes (Performance Improvement). Supports cost savings initiatives. Has active role in assessment and selection of cost effective pharmaceuticals. Reviews formulary addition request and develops monographs for presentation. Manage the formulary system and procedures for acquisition of NF product.

Consistently works and problem solves independently and effectively. Takes charge when necessary and is looked to for direction. Makes the right decisions and uses good judgment. Cross trained and multifunctional maintaining competency in multiple areas.

Manages/Mentors clinical staff pharmacists and supportive personnel. Delegates appropriate/reasonable tasks and evaluates performance. Represents Pharmacy Department at P&T Committee and Service Line Meetings.

Performs computer order entry in an accurate, thorough and timely manner. Reviews/updates medication profiles (therapies, allergies, height, weight, etc) and is cognizant of patient charges. Reviews medication orders for therapeutic appropriateness, duplications, interactions, overall effectiveness, and safety. Seeks clarification if necessary and documents interventions (i.e. ADRs).

Actively enhances personal growth and development by participating in educational programs, literature searches, in-service and meetings. Actively participates in new employee orientation and training and evaluations.

Duties performed may include one or more of the following core functions: a) Directly interacting with or caring for patients; b) Directly interacting with or caring for human-subjects research participants; c) Regularly maintaining, modifying, releasing or similarly affecting patient records (including patient financial records); or d) Regularly maintaining, modifying, releasing or similarly affecting human-subjects research records.
