Job Summary Mission Hills Post Acute in San Diego, CA is seeking a PRN Occupational Therapist to join our in‑house therapy team. Key Responsibilities
Effectively screen/evaluate patients with cognitive and/or ADL physical functioning disorders and develop appropriate plan of care following all regulatory and clinical practice standards.
Utilize standardized assessments and evidence‑based practice to support clinical interventions.
Provide rehabilitative, skilled and medically necessary treatment intervention to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.
Administer procedures as part of the rehabilitation plan including ADL retraining, therapeutic activities, therapeutic exercises, modalities, use of support and assistive devices, etc.
Assess patient progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
Evaluate treatment outcomes for effectiveness. Modify services based on patient evaluation and refer as necessary.
Provide consultation and counseling to patients, families, caregivers and other service providers related to the physical disorders.
Communicate effectively with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
Provide comprehensive discharge summary of services provided per regulatory and clinical requirements.
Qualifications
Licensed Occupational Therapist.
