Job Description The Patient Navigator provides assistance to patients and family members in the assigned area. Based on physical, mental, and social assessment skills, the Navigator works in collaboration with staff and physicians to coordinate appropriate referrals and resources to meet the needs of the patient during active treatment and upon discharge. The role functions as a liaison between acute and sub‑acute providers, incorporating assistance with care needs post‑discharge, and promotes positive patient outcomes following discharge. The Navigator provides education and emotional support, and coordinates efforts to prevent readmissions, ensuring quality delivery of care at all levels. Responsibilities include, but are not limited to, developing, collecting, and analyzing data into specific dashboards to enhance coordination of patient care.

Responsibilities

Provide individualized, appropriate care in collaboration with staff and develop patient‑specific plans of care.

Assist patients, families, and significant others in determining treatment and rehabilitation goals.

Collect specified data to evaluate quality of care and facilitate patient throughput in the admission/discharge/transfer process.

Serve as a clinical resource to all interdisciplinary team members.

Communicate and coordinate critical information related to risk issues to ensure patient safety in acute and sub‑acute settings.

Perform physiologic and psychosocial assessments to develop individualized plans of care based on patient needs.

Prioritize delivery of culturally, ethically, and spiritually appropriate care and education.

Plan and coordinate routine transitional health‑care needs, including end‑of‑life discussions and options.

Adapt education and information to individual patients and families, modifying teaching strategies as needed.

Advocate for patient rights, identify potential conflicts, and evaluate outcomes to revise plans of care.

Delegate and request assistance from interdisciplinary team members to coordinate patient needs during treatment and discharge.

Document interventions and referrals in patient charts and follow up as indicated.

Collaborate consistently with health‑care team members, patients, and families to maximize resources and outcomes.

Communicate and collaborate with community resources to enhance continuum of care for all patients.

Maintain knowledge of program initiatives related to the geriatric population and incorporate team outcomes into practice.

Provide education to staff team members on developmental needs and limitations of the geriatric population.

Qualifications

3 years of experience in an acute clinical setting with a relevant population.

Bachelor's degree in Nursing.

Proficient communication skills in English (verbal, written); computer skills.

Current and unrestricted Louisiana RN license; BLS certification.
