Location : We are hiring RNs to serve the vicinities: Northern Virginia (Leesburg, Ashburn, Alexandria etc), Tidewater (Newport News area), and Southwest Virginia (Roanoke, Blacksburg, Christiansburg, etc).

Travel Requirement : This role requires frequent travel—usually 3–5 home visits per day, driving several hours each day.

Key Responsibilities

Conduct home visits to assess and create personalized care plans.

Train and support caregivers, ensuring they can meet their loved ones' needs.

Monitor and report on clients’ health, coordinating with their primary care physicians.

Facilitate smooth transitions and ongoing care for clients.

Maintain compliance with all relevant regulations and standards.

Full‑time Hybrid position offering autonomy while utilizing nursing assessment, care planning and case‑management skills.

What You Will Bring

Must hold an active Registered Nurse (RN) License in the state of employment.

Minimum two (2) years of experience in direct care of elders or adults with disabilities.

Valid driver’s license and automobile insurance; ability to travel within assigned geographic area.

Experience in case management, care planning and assessment for eldercare, people with disabilities, and other complex medical needs in a home‑care setting.

Proficiency in common software and web‑based applications (MS Office, case‑management systems, PCP order forms, etc).

Experience with Minimum Data Set (MDS) assessment tool is a plus.

Assess client status upon initial home visit to identify needs and develop a comprehensive case‑management plan.

Use basic teaching principles to counsel and instruct patients, families and caregivers in meeting nursing and related needs.

Document accurately and timely; maintain a clinical record for each patient.

Complete the MDS‑HC and other nursing assessments while coordinating all other applicable clinical assessments.

Train, evaluate and supervise caregivers.

Monitor the health status of all members to ensure needed care is properly delivered.

Additional state‑specific qualifications may be required.
