Purpose of Position Performs a comprehensive nursing assessment upon admission of the patient including physiological, physiopsychological and environmental factors as well as health history to provide for the safety and quality of care treatment or services. Integrates all pertinent data gathered on assessment and identifies problems that form the basis of a plan of care, adhering to case management objectives.

Responsibilities

Maintain annual HHRG values at the National Standard as appropriate.

Utilize HHRG values to establish visit frequency and appropriate interdisciplinary utilization.

Ensure annual outcomes meet National Benchmarks.

Refer to community resources as appropriate.

Evaluate the clinical needs of the patient; initiate, develop, implement, and revise the patient’s plan of care; address all problems in the plan of care and document accordingly.

Participate in inter-disciplinary patient case conferences; evaluate effectiveness of the plan of care.

Instruct and provide documented supervision of the Home Health Aide plan of care every 14 days.

Notify MD/other involved care providers of significant changes in patient’s condition.

Complete admission and other paperwork in a timely manner.

Record narrative documentation to reflect skilled care; documentation is clear, concise and addresses specifics of teaching, direct care and response of patient / caregiver.

Obtain and clarify valid MD orders for all care delivered.

Plan and coordinate discharges appropriately; inform patient/MD and record disposition of patient in the discharge summary; complete all appropriate discharge paperwork.

All other duties as assigned.

Successfully pass the VNA pre-hire medication test.

Qualifications

Current RN license in Massachusetts and other related credentials to meet specialty.

Minimum 1 year of nursing experience required or 6 months of nursing experience with a BSN.

CPR certification required.

Home care and IV experience, computer skills, and BSN preferred.

Success in VNA pre-hire medication test.
