Position Overview
Provides Physical Therapy services to patients as directed by plan of care in line with agency philosophy, objectives, and policies. Supervises Physical Therapy Assistants and implements PT procedures.
Responsibilities
Assist the physician in evaluating patients’ level of function, establishing treatment plan and goals.
Perform initial evaluations within 48 hours of referral and re‑evaluations every 30 days, coordinating plan of care with the physician as needed.
Prepare clinical and progress notes; update agency documentation.
Revise plan of treatment and consult the physician for changes.
Advise and consult with patient’s family and health‑care staff.
Coordinate care with agency DON and other staff.
Observe, record, and report any changes in patient’s condition; update agency on status changes.
Supervise the Physical Therapy Assistant and check in every 4–6 patient visits.
Instruct patient and/or family in safe patient management, equipment use, and home therapy programs.
Communicate effectively with patients, caregivers, families, and agency staff.
Provide quality care and report issues or concerns to the DON/Administrator.
Ensure all tasks are performed in a timely manner.
Qualifications
Licensed Physical Therapist in the state of Illinois.
At least one year of Home Health Care clinical experience.
Valid proof of insurance coverage.
Annual TB test and CPR certification.
Strong written and verbal communication skills.
Ability to prioritize multiple functions and tasks.
Willingness to travel to patients’ residences.
