Job Description The Medical Oncology Nurse Navigator (MONN) guides patients, families, and caregivers through the cancer care continuum by addressing barriers and ensuring timely access to high-quality, community-based oncology care. Using the nursing process, the MONN provides education, facilitates decision-making, and connects patients with internal and external resources. This role involves regular collaboration with departments such as scheduling, financial counseling, pharmacy, lab, social work, and clinical teams, as well as consistent engagement with physicians and practice leadership to support a seamless navigation experience. This position will administer infusions and be responsible for various office/administrative responsibilities.
Primary Duties & Responsibilities
Performs coordination of care during the full continuum of cancer care experience.
Identifies potential and real gaps in care delivery; seeks out opportunities for avoidance and/or closure.
Prepares for, organizes, and participates in the Patient Care Conference (huddle) at regular intervals; communicates interventional outcomes to full care team in a timely manner.
Performs assessment of patient risk stratification.
Performs basic psychosocial assessments and provides emotional support; delegates and/or makes referrals for additional screening and support as needed.
Develop appropriate nursing diagnoses and corresponding action plans as needed.
Supports and/or provides oral chemotherapy education, adherence, and side effect monitoring.
Provide infusion therapies to patients and monitor their care during infusions.
Holds or participates in survivorship and advance care planning visits; makes referrals as needed.
Participates in research, quality, and performance improvement, including reporting on navigation metrics.
Supports patient advocacy and empowerment.
Participate in community outreach and cancer prevention education.
Knowledgeable of practice operations guidelines, practice organization, and healthcare economics.

Qualifications
Bachelor’s prepared Registered Nurse (BSN) required.
Five (5) years work experience as a Registered Nurse in an oncology practice or clinic.
Urology or surgical experience preferred but not essential.
Knowledge, Competencies & Skills
Current knowledge of clinical best practices, quality assurance standards, and patient care management.
Strong understanding of medical models, anatomy, pharmacology, pathophysiology, and clinical diagnostics.
Proficient in applying the full nursing process, including assessment, care planning, implementation, and evaluation.
Familiarity with cancer registries, accreditation requirements, and HIPAA compliance.
Skilled in care coordination and collaboration with multidisciplinary teams.
Ability to assess health disparities and navigate culturally sensitive patient care needs.
Excellent written and verbal communication skills.
Strong interpersonal, leadership, and collaboration abilities.
Demonstrated ability to work both independently and as part of a healthcare team.
Highly organized with critical thinking and problem‑solving skills.
Ability to multitask, prioritize, and adapt in a fast‑paced environment.
Proficient in EMR systems, computer use, and online research.
Knowledge of community resources to support patient care.
Compassionate, patient‑focused attitude with a commitment to excellent customer service.
Willingness to learn, take initiative, and contribute ideas to improve patient outcomes.

Pay Range:
$40.00 - $51.00 per hour
Actual compensation offered to candidates is based on work experience, education, skill level, and geographic location. Compensation may vary depending on the state or region in which the position is located, in accordance with applicable laws.
