Speech Language Pathologist (SLP, Speech) - HomeCare
Location: 680 Main Street, Watertown (10326)
Shift: Mon‑Fri, 5‑10 visits per week; no weekend or holiday required. Coverage: Greater Waterbury area.

Job Summary Responsible for evaluating patients’ speech, language, and/or hearing disorders and developing appropriate care plans under the direction of an MD. Implements and coordinates patient care with agency personnel and community resources, supervises Home Health Aides, and maintains accurate, timely clinical and reimbursement documentation.
Responsibilities
Manages a patient caseload on a daily basis.
Evaluates the existence, nature, and severity of speech, language, or hearing disorders and develops a care plan under MD direction.
Implements plan of care to provide safe, efficient, and quality care to patients daily.
Implements plan of care for hospice patients with appropriate end‑of‑life goals, emphasizing patient safety and education.
Evaluates care plan regularly, revising as necessary to meet patient needs.
Coordinates patient care with agency personnel and community resources; initiates case conferences and hospice interdisciplinary team meetings.
Organizes and prioritizes caseload to ensure patient care needs and agency productivity standards are met.
Keeps Rehab Coordinator informed about weekly caseload and patient problems.
Demonstrates after‑hour procedures and educates patients on on‑call/after‑hours coverage.
Prepares and keeps legible records current daily, providing all written information regarding service records, visit times, statistical sheets, and reports to physicians and other personnel.
Develops written patient care goals and expected outcomes in individualized care plans.
Updates and submits written physician orders accurately and timely.
Collects and documents necessary financial and reimbursement information for each patient, communicating as required to agency personnel and reimbursement sources.
Obtains prior authorization for services when indicated, within established timeframes.
Teaches and demonstrates to Home Health Aides specific patient care responsibilities, including transfers, ambulation, and exercise programs, accounting for patient complexity and aide ability.
Teaches, demonstrates, orients, and supervises Home Health Aides, patient/family members, and other caregivers in patient care.
Qualifications
Master’s degree from an accredited Speech Pathology program.
Current Connecticut license to practice Speech Pathology.
At least 1 year of experience in a healthcare setting; home‑health care experience preferred.
Skills
Knowledge of principles and practices of Speech Pathology.
Excellent verbal and written communication skills.
Strong organizational skills.
Excellent interpersonal skills; effective interaction with patients, families, physicians, and coworkers.
Ability to manage stress and exercise independent judgment.
Other Requirements
Valid motor vehicle operator’s license, reliable personal automobile during business hours, and active automobile insurance coverage per agency policy.
