Position Type : Full Time.

Position Summary This position serves the North Olympic Healthcare Network (NOHN) patients by coordinating care management and facilitating patient‑centered, cost‑effective care specifically for identified patient populations. The Care Management Nurse customizes and supports coordination of the patient’s plan of care built around shared goals of the patient and their healthcare team. They collaborate with providers and all members of the multi‑disciplinary health care team to appropriately communicate, update, and implement patient‑centered care plans. The Care Management Nurse shares medical expertise with patients through education and complex care coordination, and with other team members through participation in multidisciplinary patient case staffing meetings and other activities.
Location Requirement : This is an on‑site position.
Essential Job Functions
ASSESSMENT AND PLANNING: Monitor condition and status of patients on assigned panels monthly or more frequently, manage high‑risk patient care populations, support patient self‑management, and promote patient care plan adherence, follow‑up, and appointment scheduling.
OUTCOME FACILITATION: Assist with specific patient needs or goals identified by the provider and make interval contact with patients regarding clinical status updates between office visits when appropriate.
CARE COORDINATION: Work with a Patient Navigator to assist patients through the healthcare system by acting as a patient advocate, facilitate access to healthcare and community resources, and ensure continuity of care with external facilities, including hospital admission, discharge, and referrals to specialty care providers.
EDUCATION: Provide direct patient health education relevant to care plans.
DATA COLLECTION AND REPORTING: Assist in the collection, reporting, and analysis of clinical and demographic data relevant to assigned patients, evaluating clinical care and utilization of resources.
DOCUMENTATION: Document patient care information accurately, concisely, and in a timely manner in written and electronic documentation systems.
PARTICIPATION: Actively participate in staff meetings, professional education, departmental activities, and assist others in applying knowledge in clinical and operational settings.
TEAM SUPPORT: Demonstrate triage and care coordination skills and engage in cross‑coverage of the nursing team as needed.
PROFESSIONALISM: Promote collaborative practice relationships with all health care professionals, safeguard confidential information, and comply with HIPAA, fraud and abuse, conflict of interest, and the Code of Conduct.
ADHESION TO OPERATIONAL DIRECTIVES: Perform other related duties as required or requested by the CMO or CEO.

Essential Job Qualifications
Education : Bachelor’s degree as a Registered Nurse strongly preferred; current State of Washington RN licensure required.
Experience : Minimum of three (3) years relevant clinical experience. Must have prior hands‑on clinical experience and demonstrate clinical knowledge and competency.
Licensure / Certification : Valid RN licensure in Washington and BLS certification.
Effective verbal and written communication with all levels of staff personnel.
Team‑oriented, collaborative, and able to work independently in a fast‑paced medical office environment.
Strong problem‑solving and time‑management skills.
Mature confidentiality with sensitive medical information and a commitment to an ethical work environment.
Ability to perform managerial responsibilities in accordance with NOHN policies and applicable law.
Understanding and responsiveness to special populations served by NOHN, including protected characteristics under federal and state law.
Regular attendance and punctuality expected; no hunt.
English language proficiency: read, write, speak English.
