In this role, you will be responsible for conducting in-home wellness risk adjustment assessments for Medicare members and other at‑risk populations. Your primary objective will be to assess the overall health and well‑being of member beneficiaries to ensure accurate and comprehensive risk adjustment coding, leading to greater value‑based care. This is a 1099 (PRN) contractor position that offers tremendous flexibility and opportunity for those balancing competing priorities.

Location : Brooklyn, New York
Responsibilities
Perform annual in‑home wellness visits and risk adjustment health assessments on a population with chronic conditions, including comprehensive assessment, physical examination, body system review, vital signs, medication history, current prescriptions, functional status, pain management, cognitive assessment, home environment review, fall risk screening, social assessment, daily living activities, mental health screening, documentation and communication.
Provide individualized quality care to the elderly population in their homes.
Deliver patient health education on topics such as pain management and medication.
Build relationships of trust with members through exceptional communication and empathy.
Assist in closing quality care gaps (i.e., screenings and labs).
Contribute enthusiastically to the enhancement of care delivery.
Commit a minimum of 30 hours per month.
Qualifications
Valid unencumbered Nurse Practitioner license for the state of practice.
Travel within the assigned job posting county up to a maximum of 55‑mile radius.
Previous in‑home risk assessment experience preferred.
Previous 1099 (PRN) experience is preferred but not required.
Three years of patient care experience preferred (primary care/adult/geriatric, EMR).
Ability to obtain additional NP licensure if asked by Advantmed.
Reliable transportation enabling travel to members’ homes within the designated area.
Strong ability to work within our EMR system.
Ability to work independently.
Bilingual is a plus.
