Job Title: IHA Per Diem Nurse Practitioner / Physician Assistant Location:

Multiple boroughs in New York City Manhattan (New York County) The Bronx (Bronx County) Brooklyn (Kings County) Queens (Queens County) Staten Island (Richmond County) Job Type Exempt, Per Diem Full-time availability required (minimum of 30 visits per week) Reports To Manager of Care Delivery Responsibilities

Conduct comprehensive health risk assessments in members\' homes Ensure quality, timely documentation (within 24 hours) Respond to charting queries within 48 hours Deliver high member satisfaction and meet health plan expectations Participate in required training and development sessions Maintain HIPAA-compliant member confidentiality Communicate effectively with clinical leadership and team members Safely navigate personal living environments, with capability to kneel / bend and lift up to 20 lbs Provide minimum of 30 appointment slots per week Qualifications

Experience Required : PCP or in-home health experience within the last 5 years Minimum 3,600 hours of relevant clinical experience Currently enrolled with Medicaid in New York Preferred : 1+ year experience conducting in-home health assessments Education Master’s or Doctorate from an accredited Nurse Practitioner program (FNP, AGNP, ANP) OR Master’s degree from an accredited Physician Assistant program Certifications & Licenses Current NP or PA license in applicable state Board certification (ANCC, AANP, or NCCPA) Medicaid ID for New York state Medicare PTAN number Active BLS certification Valid driver’s license and automobile insurance coverage Compensation & Benefits

Competitive market rate per completed assessment Mileage reimbursement (after first 30 miles per day at current IRS rate) Flexible scheduling : daytime and evening options 60–75 minutes per assessment Competencies

Expertise in electronic medical records Working knowledge of CMS and ACA risk adjustment models Accuracy and thoroughness in documentation Strong verbal and written communication Problem-solving and adaptability in a dynamic care setting Proactive collaboration with clinical teams and health plans
