Wellness Nurse PRN

Position Summary Our Wellness Nurse (LPN) plays a vital role in ensuring the health and well‑being of our residents. In collaboration with the Health and Wellness Director, this role provides high‑quality care through health assessments, medication management, and coordination with residents, families, and the care team. Responsibilities include monitoring resident health, addressing changes in condition, staff training and supervision, and promoting a safe and supportive environment that aligns with our mission to provide exceptional care.

Key Responsibilities
Perform resident assessments and reassessments according to policies, monitoring any changes in condition.
Communicate with the Health and Wellness Director about resident needs to ensure consistent quality of services.
When necessary, participate in hands‑on care, medication administration, and specific nursing tasks.
Schedule and assign caregiving and med‑tech shifts to ensure adequate coverage and compliance with staffing guidelines and department allowances.
Supervise, train, and support care team members, promoting teamwork and adherence to care standards.
Assist in the transcription of physicians' orders and the review of new physicians' orders.
Maintain accurate and up‑to‑date resident records, including care plans, incident reports, and medical documentation.
Coordinate care planning with home‑health agencies on‑site, working within community and company policies.
Address residents’ special needs, requests, and complaints.
Coordinate admissions with the sales team, including supervising move‑ins.
Understand the community's care regulations along with state‑specific regulations.
Facilitate communication between residents, families, and healthcare providers to ensure seamless care delivery.
Candidate Qualifications Education and Certifications
A valid, incumbent nursing license (LPN/LVN) is required.
High School Diploma or equivalent.
An associate degree or higher in nursing is preferred, or a combination of education and experience. Additional coursework in management is an asset.
Current First Aid and CPR license or ability to obtain.
Experience, Competencies, and Skills
At least four (4) years working in geriatric nursing, including experience with medication administration, delivery systems, and pharmacies.
At least one (1) year of relevant supervisory experience, including staff scheduling and management, is preferred.
Experience with memory care is a plus.
Excellent attention to detail and high integrity.
Excellent communication skills and teamwork mindset.
Strong leadership and team management skills.
Outstanding organizational skills.
Positive attitude, empathy, patience, and commitment to treating our residents with dignity and respect.
Ability and willingness to participate in on‑call rotation.
