Join Otterbein SeniorLife as an MDS Nurse Transition Leader. This role serves as the MDS Nurse Leader when an opening occurs at any Otterbein location in the MDS Nurse position. The leader will complete MDS Nurse duties at the assigned location and, when an interim role is not needed, will be assigned special projects by the VP Clinical Quality.

Responsibilities
Effectively complete the MDS and care plans.
Discuss elder observations/concerns with the appropriate coordinator.
Generate/update the monthly MDS calendar and notify support staff.
Provide coaching and direction to staff as needed.
Discharge planning for residents/elders, including home services upon discharge.
Complete certifications and recertification for residents/elders.
Facilitate the weekly Medicare/therapy utilization review meetings.
Act as the therapy liaison.
Provide training materials to staff and team when appropriate.
Collaborate in the development of new processes, forms, and systems.
Be prepared and actively participate in morning meetings.
Assist in the management of the survey process when required.
Perform any other duties as assigned.
Qualifications
Education: BSN.
Licensure/Certification: Current OH RN license; RN license in IN is a plus.
Experience: Minimum 5 years of MDS experience in a long‑term care/nursing home environment is required. Experience in post‑acute environments is a plus.
Special Skills and Requirements
Proven track record of strong MDS skills (case mix management, PDPM management, quality measures).
Ability to be flexible and adapt quickly to change.
Experience with electronic medical records; PCC experience highly preferred.
Detail‑oriented with excellent literacy, documentation, communication, and interpersonal abilities; must cope with mental and emotional stress related to working with ill, disabled, elderly, and/or emotionally upset individuals.
Must be able to communicate in English.
