Nurse Practitioner/Physician Assistant (ISNP) Primary Care
Eventus WholeHealth delivers an integrated model of care to adults who reside in skilled nursing and assisted living facilities through a network of healthcare providers including Physicians, Nurse Practitioners, Physician Assistants, Medical Assistants, and in-house Support Staff.
Essential Duties And Responsibilities
Provide primary‑care services in accordance with Eventus WholeHealth site‑specific protocols and the standard of care for family medicine, geriatric medicine, and/or internal medicine as it pertains to long‑term care facility work.
Quality/Compliance
Review and sign MARs as requested by facilities, sign all orders specific for Primary Care while at the facility and within 24 hours if faxed or emailed (phone orders, DME orders, Death Certificates, Plans of Care, PT/OT orders, hospice orders, etc.). Review and address labs within 24 hours of receipt. Manage all narcotic prescriptions for the facility’s patients in accordance with Eventus WholeHealth Narcotic Rx Policy. Provide adequate time each visit for paperwork and administrative duties.
Complete documentation and submit billing records to Eventus WholeHealth per published charting and bill sheet policies. Provide the facility a hard copy of the electronic documentation within 48 hours of service for inclusion in the patient’s medical record.
Establish and maintain open and positive communication with facility staff and administration. Provide verbal and/or written instruction or feedback regarding medications and other pertinent caregiver information. Meet with the administrator or designated care coordinator prior to starting and ending each visit.
Be knowledgeable of and adhere to Eventus WholeHealth standards, policies and procedures, all legal and regulatory agencies’ rules and guidelines, and professional ethical standards. Comply with all regulatory agencies governing health care delivery.
Maintain patient confidentiality at all times, including appropriate use of cell phone, email, text messaging, patient charts and EMR. Agree to abide by and be knowledgeable of HIPAA rules and regulations.
Consult with the clinical supervisor before refusing a resident, family, or facility request or recommendation regarding the care of a resident.
Qualifications/Education
Master’s degree in Nursing from an accredited institution and a current, valid nursing license issued by the state in which you reside. All other required certifications must be current and valid.
Or:
Graduate from a physician assistant educational program accredited by the National Commission on Accreditation of Allied Education Programs.
Pass the national certification examination of the National Commission on Accreditation of Certification of Physician Assistants.
Licensed by the state in which you reside as a Physician Assistant.
Specialization in geriatric, family, or psychiatry desired.
