Job Summary:

Under indirect or direct supervision and based on understanding of patient variables such as age, sex, and other physical characteristics, monitors or manages medication therapy of hospitalized or ambulatory patients as assigned; confers with and assists medical personnel in patient care; provides medication information and pharmacokinetic dosing advice; adjusts doses, orders laboratory tests, and administers medications according to established policies and procedures; monitors medication distribution; dispenses, compounds, and distributes pharmacy products; provides direction to ancillary pharmacy staff; performs other duties as required.

Responsibilities

Uphold Kaiser Permanente policies and procedures, principles of responsibilities, and applicable state, federal and local laws.

Perform sterile and non‑sterile compounding.

Collaborate with the health‑care team to develop and implement patient‑specific plans to optimize medication therapy.

Monitor medication therapy to assure optimum therapeutic results.

Manage and optimize medication therapy based on data interpretation, assessments, and evaluations of patient outcomes.

Follow hospital‑approved collaborative, pharmacokinetic drug therapy management protocols.

Interview patients and caregivers to obtain information on medication use and allergies; incorporate that information into the medication management plan.

Document patient information appropriately in the medical record or other computer systems.

Advise patients and caregivers regarding medication use, storage, coordination with diet, compliance, precautions, warnings, and medication‑related devices.

Provide pharmacokinetic dosing advice to medical personnel.

Order and adjust medication dosages, order laboratory tests, and administer medication under protocol according to policies and procedures.

Monitor effectiveness and accuracy of the hospital medication distribution system.

Review all medication orders for appropriateness and adherence to the drug formulary.

Clarify orders and intervene when necessary.

Verify accuracy of medication preparation and dispensing by pharmacy personnel.

Provide medical personnel with medication information.

Identify potential medication safety issues and actively prevent medication errors.

Review patient records to detect adverse drug events and assist in treatment planning.

Evaluate adverse drug events for severity, probability, and preventability for review.

Verify proper delivery and correct administration of medication.

Ensure appropriate medication storage and security.

Participate in investigational drug studies as required.

Participate in problem identification, resolution, and consult supervisors when appropriate.

Support a collaborative labor‑management partnership environment.

Report discrepancies to appropriate medical and/or administrative personnel.

Develop and test requirements and specifications for protocols, computer programs, pharmacokinetic dosing, and other pharmaceutical care applications.

Participate in assigned education and training programs.

Develop and maintain teaching curriculum; conduct educational programs for pharmacy, other medical personnel, and patients.

Actively participate in rounds, hospital committees, utilization review, quality management programs, and other initiatives as needed.

Act as a member of the interdisciplinary team during medical emergencies (e.g., Code Blue).

Experience

Minimum 1200 hours of inpatient pharmacy experience within the last 5 years (e.g., intern pharmacist, pharmacist) or successful completion of an ASHP accredited clinical pharmacy practice residency.

Education

Degree in Pharmacy meeting minimum education requirements for licensure.

License, Certification, Registration

Controlled Substance Utilization Review and Evaluation System registration required at hire.

Pharmacist License (California) required at hire.

National Provider Identifier required at hire.

Advanced Cardiac Life Support within 3 months of hire from American Heart Association.

Basic Life Support within 3 months of hire from American Heart Association.

Additional Requirements

Demonstrates knowledge of patient variables (age, sex, physical characteristics) and disease states as they relate to medication therapy.

Comprehensive knowledge of acute care medications.

Knowledge of current pharmacological/biopharmaceutical principles, medical terminology, pathology, disease states, and application to patient management plans.

Experience performing aseptic technique and preparing sterile IV products and chemotherapeutic agents.

Excellent verbal and written communication skills.

Knowledge of federal and state laws and regulations regarding the provision of pharmaceutical services.

Passage of a standardized competency assessment or current incumbent in job codes 60002, 60007, 67130, or 67150.

Ability to extract pertinent information from patients or other sources and relate it to medication management plans.

Experience operating pharmacy computer systems and related equipment.

Comprehensive knowledge of emergency medications.

Preferred Qualifications

Doctorate of Pharmacy, Pharm.D.
