Summary The John D. Dingell VA Medical Center in Detroit is seeking a full-time Home-Based Primary Care (HBPC) Medical Director Board Eligible/Board Certified in either Internal Medicine or Family Medicine. Our hospital is closely affiliated with Wayne State University School of Medicine with campuses adjacent to each other. The VA Hospital serves a primary partnership role in medical education of students, residents, and fellows as well as in clinical and basic medical research.

Qualifications To qualify for this position - you must meet the basic requirements as well as any additional requirements listed in the job announcement. Applicants pending the completion of training or license requirements may be referred and tentatively selected but may not be hired until all requirements are met. Currently employed physicians in VA who met the requirements for appointment under the previous qualification standard at the time of their initial appointment are deemed to have met the basic requirements of the occupation.

Basic Requirements

United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy

Doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine

Degree obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed

Current full and unrestricted license to practice medicine or surgery in a U.S. State, Territory, or Commonwealth (or District of Columbia)

Residency training: Physicians must have completed residency training approved by the Secretary of Veterans Affairs in an accredited core specialty program leading to eligibility for board certification

Approved residencies are:

• Those approved by the Accreditation Council for Graduate Medical Education (ACGME)

• Those approved by the American Osteopathic Association (AOA)

• Other residencies (non-U.S. programs followed by a minimum of five years of verified practice in the U.S.) deemed appropriate by the local Medical Staff Executive Committee

Residents currently enrolled in ACGME/AOA accredited residency training programs and who otherwise meet the basic requirements are eligible to be appointed as “Physician Resident Providers” (PRPs)

PRPs must be fully licensed physicians (not a training license) and may only be appointed on an intermittent or fee-basis

PRPs are not considered independent practitioners and will not be privileged

They are to have a “scope of practice” that allows them to perform certain restricted duties under supervision

Additionally, surgery residents in gap years may also be appointed as PRPs

Proficiency in spoken and written English

Preferred Experience: 3 years experience working in a Home-Based Primary Care setting (Reference: https://www.va.gov/ohrm/QualificationStandards/)

Physical Requirements: To be able to do all the duties for the Primary Care HBPC Medical Director.

Duties

Provide HBPC targeted to frail, chronically ill, or disabled Veterans requiring interdisciplinary health care teams for continuity and coordination of care.

Ensure comprehensive, longitudinal, interdisciplinary home care services to maximize or maintain function, minimize institutionalization, and maintain quality of life.

Reduce the need for hospitalization, nursing home care, emergency, and outpatient visits by providing early intervention and continuous monitoring.

Promote a safe therapeutic home environment through coordinated home and community-based services.

Recognize and support the caregiver by addressing caregiver burden and providing coping resources.

Enhance the Veteran’s quality of life, especially toward end-of-life, by respecting preferences, managing symptoms, and supporting palliative care at home.

Assist the HBPC team with initial bereavement care and referral to community support if needed.

Promote a network of qualified home care professionals and provide an academic & clinical setting for research and training in interdisciplinary delivery of home-based primary care.

Serve as the overall medical care authority for the HBPC team, responsible for leadership, clinical activities, and policy development.

Collaborate with the HBPC Program Director for leadership, educational and clinical experience planning, continuous performance improvement, advocacy with VHA leadership, partnership development with PACTs, team member selection, and clinical input/oversight for patient treatment plans.

Be readily accessible for urgent consultations or physician orders during home visits.

Ensure medical care plan oversight at HBPC team meetings, either through active participation or delegation of a team physician.

Arrange physician coverage and communicate coverage plans to the HBPC team.

Act as backup for other HBPC primary care providers and make routine home visits.

Carry an active patient panel and collaborate with other HBPC Medical Directors and VISN leadership on program development issues.

Work schedule: Monday–Friday 8:00 AM–4:30 PM.
