The Community Support Worker (CSW) provides essential, community-based support to individuals receiving mental health services. The CSW helps clients achieve recovery goals, maintain housing stability, develop daily living skills, and enhance overall wellness and independence. Key Responsibilities
Provide individualized, community-based support and skill-building services according to client treatment plans. Assist clients in accessing and maintaining essential needs such as housing, employment, healthcare, and benefits. Collaborate with clinicians, case managers, and other service providers to ensure continuity of care. Document services accurately and promptly per program and agency requirements. Conduct regular home and community visits to monitor client progress and safety. Support crisis intervention and de-escalation when needed. Encourage clients’ engagement in treatment and community integration activities. Qualifications
Required: High School Diploma or GED At least one (1) year of experience providing community-based mental health or human services support Reliable transportation and valid driver’s license Ability to pass background checks Preferred: Associate’s or Bachelor’s degree in Social Work, Psychology, Human Services, or related field Familiarity with trauma-informed and recovery-oriented practices Skills & Attributes
Strong interpersonal, organizational, and communication skills Compassionate, dependable, and culturally competent Able to work independently and as part of a team Proficient in documentation and use of electronic systems Compensation
