Remote - must live in or near Vaya's Eastern North Carolina counties: Rockingham, Rowan, Alamance, Caswell, Vance, Franklin, Chatham, Person, Granville, or Stokes. Incumbent must reside in North Carolina or within 40 miles of the NC border. This position requires travel in the community and visits to our members. General Statement of Job

The primary purpose of this position is to serve individuals with serious mental illness (SMI) who are being released from a Department of Adult Corrections (DAC) facility and prioritized for re-entry coordination by both DAC and the applicable LME/MCO. The role ensures communication between all parties (DAC, health services, re-entry, community supervision) and community behavioral and physical health providers. The Justice Reentry Peer Support Specialist will coordinate and work alongside the Justice Reentry Coordinator to ensure referrals to medication management, housing support, social determinants of health needs, behavioral and physical healthcare, and benefits coordination. The Peer Support Specialist will assist with follow-up needs and coordination of services for individuals in the DAC Community Reentry Program. Referrals are made by DAC Social Workers to the Justice Reentry Coordinator and Peer Support staff approximately 90 days prior to release from incarceration. This position enables Local Management Entities/Managed Care Organizations (LME/MCOs) to interact with individuals with SMI who are within 90 days of release from a DAC facility earlier in discharge planning to support positive outcomes. The position provides case management and assistance to referred individuals. The Justice Reentry Peer Support Specialist will follow an individual indefinitely or until they qualify for tailored care management or other service providing a Therapeutic Case Management (TCM) equivalent, such as Assertive Community Treatment. The initial service period is ninety (90) days with the option to continue until needs are met as noted above. This role promotes an efficient, clinically sound, and person-centered approach to supporting individuals through this process. Contact with community stakeholders is significant; the Peer Support Specialist is responsible for ensuring high-quality customer service, including strong collaborative relationships with providers and key stakeholders (e.g., DAC, Hospitals, DSS, ACHs, behavioral health and physical health providers, internal departments, and natural supports). This is a mobile position with work in a variety of locations. The PSS staff spends substantial time in the field and completes required documentation/paperwork at an office or in the employee's home as applicable. Essential Job Functions

Managed Care Treatment Planning and Care Coordination: Possess knowledge of the needs of individuals with mental illness and substance use/addiction disorders and work with all individuals respectfully on a level they can understand Ensure all State mandated tasks related to Dispositions and discharge are completed within benchmark timeframes Ensure timely and thorough documentation of all efforts, both individual-specific and programmatic, as requested by Supervisor Coordinate with key providers to address individual needs and ensure appropriate treatment planning and evaluation Promote person-centered practices, including use of person-centered language and tools in planning Identify gaps in services and intervene to ensure appropriate care and discharge planning Focus on achieving positive outcomes, including reduction in high-risk events and inappropriate service utilization Ensure services are coordinated across the Vaya Health system and with other systems, including primary care; refer to other specialists for timely assessment as clinically indicated Provide support and direction for the individual and treatment team to ensure quality service and community integration Educate others about housing options/resources and how to access them Collaboration: Partner with individuals/families, providers, and community stakeholders to create solutions to issues, barriers to progress, etc. Work with other Vaya Health departments to address identified needs and opportunities to improve member outcomes Work with considerable independence, with most work in the community; formal supervision is monthly and informal supervision as needed This is a mobile-based position primarily during normal business hours. The employee must have and maintain high-speed internet access at home (minimum 10 Mb/s). Internet installation and monthly maintenance are the employee's responsibility. Driving is an essential function; travel within the community is required and after-hours meeting attendance should be expected on occasion. Licensure/Certification Required: North Carolina Certified Peer Support Specialist (NCPSS) Certification Preferred Work Experience: Experience with DAC populations is preferred.
