Drug and Alcohol Counselor role with Lifepoint Health. This position emphasizes service coordination, assessments, and counseling in community and clinical settings. Base pay range

PACT Team, Full-Time, Day How You'll Contribute
Provide service coordination (case management) for assigned consumers in the community (outside of traditional office settings) and provide on-call crisis coverage. Conduct comprehensive assessments and provide ongoing monitoring of mental illness symptoms. Act as liaison and consult with community agencies and families to maintain coordination in the treatment process. Coordinate with psychiatric inpatient service providers when indicated to ensure appropriate utilization of lengths of stay; take a lead role in rehabilitation services. Coordinate with outside services when detoxification is necessary and establish linkage to outpatient self-help programs and residential facilities. Provide individual and group substance use counseling. Provide ongoing assessment, problem solving, skills training, supervision, and environmental adaptations to assist clients with activities of daily living (ADLs). Why join us

High school diploma or equivalent, or combination of education and experience required for the job Bachelor's degree in social work, psychology, sociology or related field preferred 1+ year of experience BCLS upon completion of hospital orientation Bilingual and/or English-Spanish speaking preferred and ability to work with culturally diverse population Connect with a Recruiter
