POSITION SUMMARY:

The

Wraparound Care Coordination Supervisor

is responsible for oversight of services for youth who are assigned to the High Fidelity Wraparound Tier of service within Pathways to Success. Responsible the selection, training and supervision of Wraparound Care Coordinators. Provides clinical consultation to program staff regarding clients as needed concerning program services. Assures compliance with HFS Care Coordination and Support Organization requirements and all regulations in which the services and agency are provided under.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES In coordination with Manager of Care Coordination, interviews, selects, trains and supervises Wraparound Care Coordinators. Monitors employee performance and completes quarterly and annual evaluations. Works to resolve any performance issues or concerns. Provides oversight to client referrals, intake and admissions, assuring timely response to referral sources. Maintains positive relationships and open communication with community providers and others with whom the agency collaborates on behalf of clients. Oversees record keeping and reporting processes to insure that flow of written communication is current and effective. Provides leadership to the Utilization Review committee to ensure that the process is completed and that results are used to improve the quality of services. Participates in the all required committees including but not limited to monthly staff meeting and leadership meeting. Responsible for all program reviews and the success of the programs key performance indicators. Ensure needs of clients are met and services maintain Wraparound fidelity. Ensure staff have a variety of skills and resources to utilize when working with youth and families. Monitors direct service requirements of staff to ensure targets are being met and monitors caseload size to ensure accurate ratio. Adheres to and follows all other required duties necessary and assigned by Manager.

QUALITICATION REQUIREMENTS Strong organizational and time management skills. Excellent oral and written communication skills. Excellent interpersonal skills. Must possess the experience, personal qualities and practice skills to work effectively with professionals in other organizations and from other disciplines and to provide services consistent with Systems of Care and Wraparound. Ability to supervise and support staff both onsite and remote locations. High level of discretion with the ability to handle confidential and sensitive information.

Completion of CARE and TCI training is an internal requirement for all staff

and must be completed upon hire. Ongoing application of these principles is expected and essential to successful job performance.

TCI (Therapeutic Crisis Intervention). Employees must: Pass the written test annually Pass the physical test every six months (if applicable to their role) Monthly refresher opportunities are provided. Employees who fall out of compliance may be required to retake the full course. Consistent participation and application of both CARE and TCI are essential to job performance.

Working Conditions This position operates in a professional office environment and routinely uses standard equipment including, but not limited to, filing cabinets, computers, phones, photocopiers, etc. The role may involve frequent walking, standing, or lifting of materials up to 10 pounds, depending on program needs. These physical demands are representative of those needed to perform the essential functions of the job.

Requirements:

EDUCATION AND/OR EXPERIENCE A Masters Degree in Social Social Work, Counseling, or Psychology from an accredited program, or in a related human service field. License of Clinical Social Work (LCSW) or Licensed Clinical Professional Counselor (LCPC) preferred. Supervisory experience in the human services field is required Knowledge of community programs, services and evidence-based practices and a variety of treatment modalities used to work with youth and their family to navigate child serving systems Requirements Must meet DCFS/CANTS Clearance Requirements. Must be at least 21 years of age. Must possess and maintain a valid driver’s license and maintain proof of automobile insurance.
