Overview

FACETS is seeking a mission-driven Case Manager to join the Next Steps Family Program in region 1 of Fairfax County. The role provides housing-related services to families experiencing homelessness or at risk of homelessness, in an emergency shelter and prevention program located in scattered-site apartments in Alexandria, VA, with aftercare for those recently exited from Shelter and homeless prevention/diversion services in the community. Bachelor’s degree required; MSW and Bi-lingual (English/Spanish) are a plus.

Duties and Responsibilities

Assess housing barriers of families who are homeless or at-risk of homelessness to determine housing and service needs.

Develop housing procurement, financial, and self-sufficiency case management plans with participants, including intake interviews to determine household needs, goals, and eligibility.

Apply the Housing First model to provide housing stabilization services, prevent homelessness, and divert households from emergency shelter.

Provide mediation and advocacy with landlords to develop a plan to obtain and/or maintain housing; educate tenants on rights and responsibilities.

Maintain consistent verbal and written communication among operations staff, tenants, landlords, referral sources, collaborating agencies, debtors, and creditors.

Determine appropriateness of households for rapid rehousing funds and track fund use.

Advocate for and assist families in obtaining services (e.g., mental health, housing referrals, financial assistance, home-based services, training, medical services, mentoring, socialization).

Conduct weekly home inspections for Next Steps Family Shelter participants and teach households how to maintain an apartment.

Travel for shelter and home visits and work an adjusted schedule including evenings and weekends as necessary.

Use HMIS and other records to maintain, update, and report case data, goals, and outcomes in a timely manner.

Participate in program staff meetings and work cooperatively with staff to meet FACETS goals.

Represent FACETS in the community in line with Agency Mission, Vision, and Code of Ethics. Perform other duties as assigned.

Required Skills/Abilities

Bachelor’s Degree in a human service-related field.

Social work/case management experience with homeless and/or at‑risk populations or experience in an emergency shelter setting, or similar case management experience.

Knowledge and belief in the Housing First and Rapid Re-Housing philosophies and strategies.

Understanding of tenant rights and responsibilities and strengths-based case management.

Ability to work independently in a fast-paced environment and manage a caseload with minimal supervision.

Excellent data entry and data quality skills; ability to use HMIS to establish and maintain case records and facilitate data collection.

Clear and concise communication, both oral and written.

Proficiency with Microsoft Office Suite; ability to work a flexible schedule and manage workload.

Must pass criminal background and Child Protective Services background checks.

Physical Requirements

Prolonged periods of sitting at a desk and working on a computer.

Must be able to lift 15 pounds occasionally.

Occasional travel to other FACETS office locations within Fairfax County and participation in annual fundraising events.

Salary and Benefits Salary: $47,000.00 – $50,000.00 per year. FACETS offers a competitive salary commensurate with qualification/experience. Benefits include observing 11 federal-paid holidays, with prorated pay when holidays fall on a scheduled workday, and reimbursement of work-related expenses such as mileage and cell phone/data expenses. FACETS provides ongoing professional development and training.
