In this vital role, you’ll oversee the social services department and ensure that residents’ emotional, social, and discharge planning needs are met with professionalism and empathy. The ideal candidate will have strong leadership skills, a deep understanding of resident rights and care coordination, and the ability to collaborate effectively with residents, families, and interdisciplinary team members.
Responsibilities
Manage and support the social services department to provide exceptional care and service to residents.
Conduct assessments and develop individualized care plans in coordination with clinical teams.
Guide residents and families through admission, adjustment, and discharge processes.
Ensure compliance with federal, state, and facility policies related to resident care and rights.
Maintain accurate documentation and participate in care plan meetings.
Qualifications
Bachelor’s degree in Social Work (BSW) or related field required.
Current Connecticut licensure (LSW or LMSW) preferred.
Previous experience in a skilled nursing or long-term care setting strongly desired.
Excellent communication, organization, and leadership skills.
