Advanced Practice Provider (Nurse Practitioner or Physician Assistant)
to join our
Family Practice team
in
Bourbonnais, Illinois . This is a full-time opportunity to provide high-quality, patient-centered primary care in a community-focused outpatient clinic.
Responsibilities
Full-time, outpatient primary care role
Provide care across the lifespan, focusing on prevention, chronic disease management, and acute concerns
Collaborate with physicians and support staff in a team-based care model
Build meaningful relationships with patients in a supportive community setting
Qualifications
Licensed Nurse Practitioner or Physician Assistant in the state of Illinois
Board certification in Family Medicine (NP) or relevant PA specialty
Excellent communication and clinical decision-making skills
Passion for delivering high‑quality, compassionate care

Pay Transparency Prime Healthcare Illinois Medical Group offers competitive compensation and a comprehensive benefits package that provides employees the flexibility to tailor benefits according to their individual needs. Our Total Rewards package includes, but is not limited to, paid time off, a 401K retirement plan, medical, dental, and vision coverage, tuition reimbursement, and many more voluntary benefit options. A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $110,000.00 to $125,000.00 on an annualized basis. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
