Case Manager II Our Epilepsy Mental Health Services team is seeking a Case Manager II to work with clients who are diagnosed with epilepsy. We support people in connecting with resources to live as independently as possible. We help them develop self‑awareness to make positive plans for the future through frequent meetings in the community. Case Managers develop long‑term relationships with their clients and operate autonomously, creating their own schedules. Caseloads between 10–15 on average.

Hours:

Full‑Time, Monday – Friday flexible business hours

Location:

Twin Cities Metro Area, Minnesota | Hybrid – in office and remote opportunities available

Range:

$24.00–$25.35/ Hourly, dependent on qualifications

Hiring Bonus:

$1,000 to external candidates

Main Job Duties

Attending client meetings in‑person as needed

Coordination of provider care and transportation as needed to appointments

Assessing client needs and ensuring continuity of care

Making ongoing referrals to internal and external resources based on assessment and current client need

Maintaining necessary daily records for continuity of service and reports to funding agencies

Conducting outreach and building sustainable relationships with our partnering organizations

Requirements Required Qualifications

Valid driver's license, excellent driving record and current auto insurance

AND EITHER

Bachelor's Degree in behavioral science or related field

2,000 hours of supervised experience working with adults with severe and persistent mental illness

OR

Master's Degree in behavioral science or related field

Preferred Qualifications

Master's degree in behavioral science or related field

Experience providing direct services to adults with serious and persistent mental illness

Experience with case management and commitment proceedings

Knowledge of Harm Reduction, Motivational Interviewing and Person‑Centered approaches
