Overview

The Mental Health Residential Rehabilitation Treatment Program (MH RRTP) Social Worker (Transitional Care Manager) will provide a range of direct patient care services consistent with area of assignment. The Social Worker demonstrates the ability to balance quantity and quality requirements - agreed upon priorities are maintained - and caseload is effectively managed - with evidence of acceptable work quality by satisfactory supervisory or peer case review rating. Responsibilities

Provide a range of direct patient care services consistent with the area of assignment. Engage in psychosocial and diagnostic assessments - establish therapeutic rapport - administer therapeutic interventions (including individual and group therapy) - and ensure continuity of patient care in the development and implementation of psychosocial treatment and discharge plans with clients of all ages. Establish a proactive, resourceful approach to discharge planning - including initiating, coordinating, and expediting the successful discharge of Veteran clients. Demonstrate the ability to perform and monitor quality professional psychosocial services for the young, middle age, and geriatric patients. Facilitate the admission process for veterans seeking admission into the MH RRTP (including all programming tracks) - participating in the screening process and determining which program best meets the veterans’ needs. Coordinate transitional case management duties - including coordination of consults and self-referrals to the MH RRTP and collaboration with the MH RRTP Screening Board across VISN and VA. Provide veterans with verbal and written educational materials about MH RRTP programs as appropriate. Comply with VA policy and directives ensuring completed pre- and post-admission documentation and veteran education. Maintain and communicate a schedule of MH RRTP admissions and discharges and track requirements and program initiatives and quality monitors as specified by accrediting bodies (e.g., CARF, The Joint Commission) and VA policies. Communicate effectively with VA network and community agencies to facilitate treatment, discharge planning, and continuity of care; engage in referrals to VA and community programs as needed. Demonstrate regular contact and collaboration with VA and community agencies to enhance coordination of patient care and problem resolution. Identify and address clinical risk; comply with suicide prevention policies and directives (e.g., RISK-ID); engage in suicide screening, comprehensive suicide evaluations, safety planning, and referrals or warm handoffs to meet veteran safety needs. Provide crisis intervention services and have working knowledge of medical and mental health diagnoses, disabilities, and treatment procedures. Operate a motor vehicle to transport patients off-site as part of duties. Work indoors and outdoors; exposure to weather elements may occur. Work schedule: Tuesday-Saturday 8:00am-4:30pm or Sunday-Thursday 8:00am-4:30pm. Qualifications

Basic Requirements: United States Citizenship; education – master’s degree in social work from a CSWE-accredited school; graduation from schools not yet fully accredited does not meet the requirement; doctoral degree may not substitute for the master's degree; licensure/certification by a state to independently practice social work at the master’s level; statespecific licensure requirements; VA waivers allowed for certain cases at GS-9 with time-bound licensure requirements; post-MSW experience requirements may apply in some states; knowledge, skills, and abilities (KSAs) include: ability to work with Veterans and families from diverse backgrounds; psychosocial assessment and treatment planning; ability to implement treatment modalities with individuals, families, and groups; establish effective working relationships; fundamental knowledge of medical and mental health diagnoses and treatments. GS-9/GS-11 specific requirements: One year of post-MSW experience for GS-11 with independent state licensure or certification; or doctoral degree substitution for one year of experience; additional KSAs include community resource knowledge, independent psychosocial assessments and interventions, medical/mental health knowledge, treatment modalities, and consultation to new social workers and students.
