Responsibilities

Perform an annual wellness exam for senior Medicare patients in clinic

Collaborate with Primary Care team to ensure completion of holistic patient care

Combination of home health and telehealth - 30 mile radius

3 areas available - Long Beach, South Bay, East Los Angeles

Mon-Fri 8a-5p

8-10 patients per day

Qualifications

Family Nurse Practitioner (FNP) or Adult Nurse Practitioner (ANP)

1+ yrs exp as NP required - Primary Care, Family Med, Internal Med, Gerontology, or Palliative Care exp. Home Care exp is a plus

CA license and DEA required

Board Certified or Board Eligible Medical Doctor with or w/o US residency with current US work visa
