POSITION SUMMARY – RN Case Manager

POSITION DUTIES



Provides care to patient and family care that progresses them towards self-identified, holistic, and individualized goals. Performs nursing process through individualized assessment, planning, implementation and evaluation of safe, therapeutic, efficient care for patients. Promotes quality outcomes. Collaborates or seeks additional resources as needed in the management of complex, new/unusual, or other high-risk clinical situations Serves as patient and family advocate in ensuring excellent and satisfying care.

Minimum Required Qualifications –

CA RN license CPR/BLS 3-5 years of case manager experience in an acute care facility 3+ years recent RN experience working in RN role 2+ years recent RN experience working in an specialty area Travel experience

PREFERRED QUALIFICATIONS – ACM Or CCM

LENGTH OF ASSIGNMENT

– 13 weeks

SHIFT / HOURS PER WEEK

– 8h day (variable start times: 0800-0930) – 40hrs/wk
