Overview
RN (Long Term Care Sub-Acute) – Heritage House – Troy, NY. RN position in skilled nursing within the Eddy Heritage House/Nursing and Rehabilitation Center family. 122-bed facility with sub-acute rehabilitation services, focused on quality of life in a homelike environment. This role is with Trinity Health and St. Peter's Health Partners network. Responsibilities
Utilize the nursing process to diagnose and treat human responses to actual or potential problems of individuals or groups. Conduct admission assessments in accordance with guidelines and interview residents/significant others to complete health history. Assess residents using available resources when conditions are new or not commonly seen on unit. Interpret assessment data to identify acute psychological or psychosocial risk and report/record interpretations. Inform residents about care or services to be provided and contribute to the plan of care based on identified problems and medical plan of care. Develop and communicate the plan of care with action plans, goals, and time frames. What you will need
A current license to practice as a Registered Nurse (RN) in the State of New York. Associate degree in nursing or Degree of Nursing from an accredited school. Pay range information: $33.10 - $47.75 per hour; compensation based on experience, skills, and education. Exempt under FLSA; ranges may vary by location. Employment Details
