Atlanta, GA

Hybrid:

Associates required to be in‑office 1‑2 days per week; remote work otherwise. Candidates not within reasonable commuting distance will not be considered unless accommodation is provided.

Role Overview:

The Special Programs Case Manager I delivers telephonic/virtual case management within licensed scope for special programs such as Adoption Assistance and Foster Care, managing overall healthcare costs through integrated physical/behavioral health and whole‑person care.

Responsibilities

Conduct assessments to identify individual needs.

Develop comprehensive care plans to address goals identified during assessments.

Support member access to cost‑effective care and modify plans as needed.

Coordinate with internal and external resources for integrated whole‑person care.

Address social determinants of health in care planning.

Work closely with state agencies.

Maintain knowledge of system of care philosophy and community‑based services.

Build partnerships with designated populations and families, addressing cultural and linguistic needs.

Evaluate health needs and identify applicable services in conjunction with members and families.

Provide patient education, medication reconciliation, and community resource information; assist with follow‑up care arrangements.

Minimum Qualifications

MS/MA in social work, counseling, or related behavioral health field or nursing degree with 3+ years of clinical experience in social work counseling involving complex psychiatric and substance abuse treatment.

Active, current, valid license as RN, LCSW, LMHC, LPC, LMFT, or Clinical Psychologist (state laws applicable).

Preferred Skills, Capabilities, and Experiences

Experience working with specialty populations.

Knowledge of foster care, adoption, juvenile justice systems.

Clinical experience with complex psychiatric/substance abuse and/or medical disorders.

For government business: LAPC, LAMFT, LMSW (if applicable).

Willingness to travel to worksite as needed.
