Clinical Pharmacist Specialist - Oncology at UT Health San Antonio. The role involves demonstrating proficiency in all pharmacist functions, including interpreting, transcribing, and verifying medication orders, and participating in multidisciplinary rounds, drug utilization reviews, clinical services development, and staff education.

Responsibilities

Collaborate with healthcare professionals to develop and implement individualized medication plans for patients.

Conduct comprehensive medication reviews, assessing the appropriateness of therapy and making necessary adjustments.

Provide clinical pharmacy services in various areas including pediatrics, pain management, and critical care.

Administer IV infusions and monitor patient responses to therapy.

Educate patients and healthcare staff on medication use, potential side effects, and interactions.

Participate in multidisciplinary rounds to provide pharmaceutical expertise in patient care discussions.

Document clinical interventions and outcomes in the electronic health record system.

Qualifications

Bachelor's degree in pharmacy or Doctor of Pharmacy from an ACPE‑accredited college of Pharmacy is required.

Current TX state license to practice pharmacy, or licensure in TX within 90 days of hire is required.

Basic Life Support upon hire required.

Advanced Cardiac Life Support within 90 days of hire.

Board of Pharmacy Specialty (BPS) certification preferred.
