Under supervision and in collaboration with the physician staff, the Advanced Practice Provider (APP) performs physical examinations and treats common episodic and chronic healthcare problems with emphasis on health maintenance and disease prevention. This role is designed for healthcare professionals such as nurse practitioners and physician assistants who possess advanced training and expertise. Primary responsibilities include patient care in the clinic setting, in-office procedures, and patient education. The APP contributes to high-quality healthcare delivery and patient outcomes. Job Requirements, Prerequisites And Essential Functions
Pay Range: $87.58 - $102.18 - $119.77 Essential Functions And Responsibilities
Assess patient status by taking health history through interviews and chart reviews; assess presenting illness, risk factors, family history, psychosocial, and cultural factors; and perform physical examinations. Order/perform appropriate laboratory tests and screening tests. Seek additional information as needed, including consultations with physicians and other clinicians, to evaluate illness. Integrate data to determine diagnosis and therapeutic plan, including health risks. Develop and implement treatment plans by prescribing/dispensing medications and/or injections in compliance with guidelines and state laws. Instruct patients and families, educate on health promotion/illness prevention, refer to community resources, and document in EMR. Participate in peer reviews, chart reviews, staff education, guideline development, and quality assurance activities. Act as consultant to the clinical team; comply with confidentiality; promote patient advocacy. Collaborate with physicians in managing acute and long-term medical needs; provide monitoring and continuity of care between visits; triage patient calls/emails. Elicit and document a complete health history. Perform and document a complete physical examination. Order appropriate diagnostic tests. Assess patient perception of health status and factors affecting health including psycho-social, physical, environmental, economic, and cultural factors. Identify patients and families at risk for specific healthcare problems. Diagnosis And Planning
Formulate diagnoses based on history, exam, and tests. Formulate and document care plans with patients and families, including treatment and health maintenance education. Establish short- and long-term goals with each patient. Plan comprehensive patient care with other providers. Implementation
Provide appropriate care based on patient needs and age. Implement treatment plans based on assessments and diagnostic results. Counsel patients on motivation and adherence. Provide telephone advice and triage for acute and unscheduled patients. Document all pertinent data in patient records; provide medication refills as needed. Review abnormal lab data and tests and take appropriate action; consult with other providers as indicated. Evaluation
Evaluate patient understanding and adherence to the care plan with follow-up and reassessment. Education
Patient and Family: identify learning needs, assess readiness, provide individualized education, and develop goals/objectives that meet care needs. Staff: function as a clinical resource. Continuity/Coordination
Act as the health care provider for a specified patient group. Coordinate care among providers, patients, and families; participate in discharge planning when appropriate; promote ongoing communication with community resources. Advocacy
Encourage patient and family participation in care planning; assist in obtaining information to enable informed decisions; inform patients of their rights; provide referrals to resources. Qualifications – Nurse Practitioner
BSN required; MSN preferred Successful completion of an approved advanced practitioner program and certification as a practitioner (general or specialized) License/Certifications
Current RN license in California and successful completion of an NP program Active and unrestricted Nurse Practitioner license in CA Current Nurse Practitioner Furnishing License (if applicable) Must hold national board certification by first day of work and maintain Active and transferable DEA license to CA as required by the specialty Eligible for CMS/Medi‑Cal enrollment BLS and ACLS Certification (American Heart Association) required Experience
