Simple Solutions Psychotherapy - Founded June of 2020. We are a community-based organization providing therapy (individual, family, couples, & groups), after‑school programs, mental health assemblies, organizational workshops, juvenile detention centers, case management and more! We service California residents via insurance or cash pay. Services are provided via telehealth and in‑person in English and Spanish Language and based on specifics of contracts.

At Simple Solutions Psychotherapy we believe "It's SIMPLE people just make it complicated." We seek to assist PsyD practicum students in gaining knowledge and experience toward assisting the community with mental health services.

About the role Case Managers perform several tasks to achieve the best outcomes for their clients. A Case Manager will be responsible for assessing, planning, implementing, monitoring, and evaluating the services and resources required to meet the diverse needs of our clients. These duties usually involve working with their clients directly and often working with the client’s family members, close friends, and health care workers. Case managers will need to locate resources for their caseload, resources may include assisting in finding housing, food banks, connecting to local pharmacies, etc.

What you'll do

Following company policies and procedures to assist the client with their specific needs.

Conduct thorough assessments for each individual client regarding the areas of the client’s needs, including health, psychological, social, and finances.

Create comprehensive and individualized care plans while collaborating with clients, their families, and other involved parties.

Coordinate access to necessary services, resources, and support networks for clients.

Documenting all contacts with clients using specific databases and verbiage specific to the program.

Monitor and evaluate the effectiveness of the care plan, eliminating barriers, and adjusting as needed to ensure optimal client outcomes.

Assisting clients in overcoming barriers using motivational interviewing techniques.

Maintain accurate and updated documentation of client interactions, progress, and service referrals.

Collaborate with interdisciplinary teams as needed, such as healthcare professionals, social workers, and community organizations, to ensure proper care for clients.

Participate in case conferences, meetings, and training sessions as needed or required.

● Proficiency in maintaining accurate and detailed client records and documentation.

● Proficient with Microsoft Office or related software.

● Willingness to adapt to changing situations and environments.

Qualifications ● High School Diploma or GED

● 2-3+ years of customer service experience preferred, preferably in human services, possess demonstrated commitment to customer service.

● Professional or personal experience supporting individuals with disabilities, mental illnesses, or challenging behaviors is highly preferred.

● Knowledge of appropriate software including Microsoft Word, Excel, Outlook, Microsoft PowerPoint, and Adobe Acrobat.

The pay range for this role is:

20 - 25 USD per hour (Rancho Cucamonga, CA)
