Role & Responsibilities
Conduct initial and ongoing comprehensive assessments of the patient's functional status, including mobility, strength, and home environment safety.
Develop, implement, and document the Plan of Care (POC) in coordination with the physician and clinical supervisor.
Provide skilled physical therapy treatments such as therapeutic exercises, manual therapy, and gait training to restore function and prevent disability.
Educate patients, family members, and caregivers on the safe and effective use of necessary medical equipment, home exercise programs, and safety precautions.
Complete all clinical documentation accurately and efficiently using our Electronic Medical Record (EMR) system.
What You'll Need
License: Current and unrestricted Physical Therapist (PT) license in the State of California.
Experience: 1+ year of clinical experience as a Physical Therapist (Home Health experience strongly preferred).
Certification: Current CPR certification (AHA Basic Life Support required).
Logistics: Valid driver's license, reliable and insured vehicle, and comfortable with travel across the East County service area.
Skills: Strong clinical judgment, excellent communication, and a compassionate, patient-first attitude.
