Senior Habilitation Counselor
Position: Senior Habilitation Counselor – Multiple Vacancies
Department: UBHC-Aipu/Aadh P/T Nurses-PISC
Overview Rutgers University Behavioral Health Care (UBHC), established in 1971, offers comprehensive evidence‑based behavioral health and addiction services for children, adolescents, adults, and seniors across New Jersey. UBHC employs 1,060 professionals and supports 15 sites throughout the state.
Posting Summary The Senior Habilitation Counselor provides assessments for the Children’s Mobile Response and Stabilization (CMRSS) program in Middlesex County, ensuring quality and quantity standards within UBHC and the Children’s System of Care (CSoC). Duties include crisis assessment, outreach, referrals, stabilization, and aftercare monitoring. The role also supports program coordination and resource development.
Responsibilities
Actively participate in handling telephone calls/mobile response requests in a consumer‑friendly manner.
Maintain comprehensive and up‑to‑date knowledge of eligibility and admission criteria for units and programs served by CMRSS.
Maintain comprehensive and up‑to‑date knowledge of specific guidelines of the CMRSS program.
Obtain crisis assessment information according to CMRSS protocols and input data accurately into the unit's information system.
Intervene to control or defuse crises.
Complete crisis assessment using the crisis assessment tool.
Provide relevant input in assessing suicide risk, mental disorders, and level of care determinations.
Facilitate development of an individual crisis plan (ICP).
Acquire familiarity with agencies in Middlesex County that provide crisis stabilization and ongoing support/resources for the family.
Complete appropriate documentation and utilization review.
Minimum Education and Experience
Bachelor’s Degree in a behavioral health or related human services field and two (2) years of related experience, or a Master’s Degree in a behavioral health or human services field and one (1) year of related experience.
Certifications and Licenses
Applicants with a Social Work degree must possess an active New Jersey certification/licensure as required by the State Board of Social Work.
Maintain CSoC Crisis Assessment Tool (CAT) certification and re‑certification annually.
Valid driver's license.
Physical Demands and Work Environment PHYSICAL DEMANDS:
Frequently spends prolonged time sitting, talking, listening in groups and on the telephone, walking, bending, stretching, and pulling. May need to work long hours, travel within the state, and lift up to 10 pounds. Must be able to drive and respond to a family crisis in Middlesex County within one (1) hour when on‑call.
WORK ENVIRONMENT:
Handles phone requests under time and decision‑making pressure. When in the office, generally pleasant working conditions in a clean, well‑lit, quiet to moderately loud setting.
Special Conditions
Must be available for on‑call.
