Nurse Practitioner/Physician Assistant Primary Care - PT Weekdays Position Summary

PT position with 2-3 weekday availability required.

Eventus WholeHealth delivers an integrated model of care to adults who reside in skilled nursing and assisted living facilities through a network of healthcare providers including Physicians, Nurse Practitioners, Physician Assistants, Medical Assistants and in‑house Support Staff.

Responsibilities

Provide primary‑care services in accordance with Eventus WholeHealth site‑specific protocols and consistent with the standard of care for the specialty of family medicine, geriatric medicine, and/or internal medicine as it pertains to long term care facility work.

Quality/Compliance

Review and sign MARs as requested by facilities, sign all orders specific for primary care while at the facility and/or within 24 hours if faxed or emailed, including phone orders, DME orders, death certificates, plans of care, PT/OT orders, hospice orders as appropriate, and other orders relevant to the facility patient care needs.

Manage all narcotic prescriptions specific to your facility’s patients in accordance with Eventus WholeHealth Narcotic Rx Policy.

Complete documentation and submit billing records to Eventus WholeHealth according to published charting and bill sheet submission policies, and provide a hard copy of the electronic documentation within 48 hours of service for inclusion in the patient’s facility medical record.

Maintain open and positive communication with facility staff and administration; provide verbal or written instructions or feedback regarding medications and other pertinent caregiver information.

Meet with the Administrator or designated facility care coordinator before patient care visits each day and prior to leaving the facility each day.

Adhere to Eventus WholeHealth standards, policies, procedures, and all applicable legal and regulatory agency rules and guidelines.

Maintain patient confidentiality at all times, including appropriate use of cell phone, email, text messaging, patient charts, and EMR, and abide by HIPAA regulations.

Consult with the clinical supervisor before refusing a resident, family, or facility request regarding the care of a resident.

Qualifications

Master’s degree in Nursing from an accredited institution and a current and valid license to practice in the state of residence. All other certifications necessary to perform duties must be current and valid.

OR

Graduated from a physician assistant educational program accredited by the National Commission on Accreditation of Allied Education Programs; passed the national certification examination of the National Commission on Accreditation of Certification of Physician Assistants; licensed as a Physician Assistant in the state of residence; specialization in geriatric, family, or psychiatry preferred.
