Speech Language Pathologist (SLP) in Skilled Nursing Facility
Flagstar Rehab SLPs work with a diverse and multidisciplinary team to perform evaluations, therapies, and patient/family education to restore speech and swallow abilities, improving patients’ daily functions.
Job Duties
Provide efficient speech or swallow evaluations and/or screen patients.
Develop care plans, down‑grade or upgrade as appropriate, manage meal textures.
Deliver quality therapy services to individuals in an SNF setting for conditions such as TBI, CVA, stroke, Parkinson’s, multiple sclerosis, cancer, orthopedic surgery, and other debilitating diagnoses.
Monitor and document patient progress using innovative EMR software.

Not accepting Clinical Fellows (CFY) at this time.
Current NYS SLP license and registration.
Clinical Competence Certification (CCC).
CPR certificate.
EMR competence.
Meets all state and federal requirements for U.S. employment in a nursing facility.
Malpractice/litigation insurance (if applicable).
