Responsible for delivering nursing care to residents/patients in a long-term care facility under the supervision of the Nurse Manager, Nurse Coordinator, and/or RN. Recognize changes in residents'/patients' conditions and respond appropriately. Communicate effectively with the nurse manager and other members of the healthcare team regarding patient/resident needs and/or changes in condition. Appropriately assign duties to Certified Nurse Assistants based on resident/patient needs, staff availability, and unit needs. Experience in nursing home, nursing, LPN, long-term care LPN, subacute LPN, licensed practical nurse. Qualifications
Certificate from an accredited Licensed Practical Nurse Program or required education as described by the NYS Education Department. NYS LPN license required. Good clinical patient care skills and communication skills. Must be service-oriented with excellent interpersonal skills. Ability to read and write in English is required. Valid BCLS certification. Salary Range
