About This Position
The Licensed Graduate Social Worker (LGSW) provides services to customers and their families to help them adjust to physical or mental illness, injury or hospitalization. Assisting the interdisciplinary treatment team, the social worker performs biopsychosocial assessments, discharge planning, psychosocial coordination, and community service referrals throughout patient hospital stays. The role includes behavioral and psychological needs assessment, case management, and discharge planning for clients assigned to the unit.
Requirements
Education : Master’s degree in Social Work from an accredited institution; working toward a license as an Independent Clinical Social Worker and fulfilling the requirements set forth by the Minnesota Board of Social Work.
Experience : Minimum of 4,000 hours of post‑graduate clinical social mental health professional experience (as defined by Minnesota statute). Hospital social work experience preferred. Knowledge of current case management principles, length‑of‑stay management, and/or transition/discharge planning highly valued.
Licensure/Certification : Licensure at the LGSW or LICSW level in the state of Minnesota; valid Class D driver’s license for home‑health duties.
