The Pediatric School-based Nurse (LPN) will be working within a Multi-disciplinary Team to provide 1:1 care to a student who has a brain injury or brain-based disorder and medically-fragile. The students are non-verbal, non-ambulatory and some have a trach, vent and/or g-tube. Most of the students have a seizure disorder. Successful attributes include a desire to change the world for millions of children who suffer from brain injuries or brain-based disorders and a strong strategic focus, individual leadership in an environment that promotes innovation and continuous improvement, effective communication with key constituents and stakeholders, and accountability for results. Providing 1:1 care to a medically fragile students Assist School Nurse in obtaining/completing all relevant medical documents including but not limited to MAFs, IHPs, care plans Effectively follow care-plans as detailed from MAFs and School Nurse to allow for following accurate feeding protocols Assist School Nurse with any/all medical needs of the students as assigned Provide basic healthcare to students in case of injury or acute illness Educate students and staff on healthy habits, such as proper nutrition and hygiene Keep track of students’ vaccination records Update students’ medical history Ensure school environment is safe for children and school staff (e.g. prevention of communicable diseases) Review school documentation pertaining to body checks and other incidents, inform parents of such incidents and complete and submit related documentation in a timely fashion. Assist in the development of IEP instructional and nursing goals/objectives; develops individualized school health plan (ISHP) as needed Maintain cooperative working relationships with parents, staff, and other school district personnel; serves as a resource/liaison with other agencies and other community services; maintains effective and timely written and oral communication with parents, Qualifications
Current NYS LPN Must be willing to travel with student to/from home (NYC)/school (NYC) One to three years of pediatric experience in hospital or school-based setting Must have excellent communication, interpersonal, and problem-solving skills. Must have the ability to take initiative, and a highly flexible personality type Must be able to lift at least 50 pounds independently. Must be detail-oriented, focused, highly motivated, and able to work as part of a team. Spanish speaking is a plus MUST HAVE A SENSE OF HUMOR.
