Oncology Nurse Navigator

with our Solid Tumor team in

Hyde Park . In this role you will partner with the rest of the clinical team to provide patients the highest quality of care.

Under the direction of a faculty sponsor and in collaboration with the program support team, the

Oncology Nurse Navigator

coordinates the care of patients through the disease process and acts as a clinical liaison for patients, across medical services and clinical departments.

Essential Job Functions:

Assess clinical, emotional, spiritual, psychosocial, financial, and other patient needs

Facilitate resolution to patient needs and monitor follow up

Educate/reinforce education with patients and families on disease, treatments, side effects, adverse reactions, and reportable signs/symptoms based on multidisciplinary care plan

Coordinate discharge planning with the patient’s family, social workers, home nursing staff, oncologists, and personal physicians

Work with patients and family members to ensure treatment instructions are understood and followed

Develop and/or coordinate development of patient education materials, clinical pathways, and quality studies

Coordinate and/or participate in multidisciplinary consultations and/or tumor conference

Document recommendations made at multidisciplinary consultations and/or tumor conferences

Required Qualifications:

Registered Nurse with a current Illinois license

Graduate of a National League for Nursing accredited School of Nursing with BSN

Preferred Qualifications:

OCN Certification

ONS Chemotherapy Provider

Minimum of two years hematology/oncology clinical nursing experience with demonstrated clinical competence in patient care, teaching and management of patients with a cancer diagnosis, and knowledge about clinical aspects of oncology including protocol therapy and data management.

Compensation & Benefits Overview

UChicago Medicine is committed to transparency in compensation and benefits. The pay range provided reflects the anticipated wage or salary reasonably expected to be offered for the position.

The pay range is based on a full-time equivalent (1.0 FTE) and is reflective of current market data, reviewed on an annual basis. Compensation offered at the time of hire will vary based on candidate qualifications and experience and organizational considerations, such as internal equity. Pay ranges for employees subject to Collective Bargaining Agreements are negotiated by the medical center and their respective union.
