Pay: $145.00/hr – $165.00/hr. (Executive pay range may vary based on skills and experience.)

Position Summary We are committed to providing quality health care for everyone in order to build a healthier community. We are currently accepting applications for a 1099 Contract Primary Care Physician.

Job Summary Under the direction of the Chief Medical Officer, the Primary Care Physician is primarily responsible for participating in the multidisciplinary process of providing care and treatment to patients. The Physician provides the full scope of primary care services, including diagnosis, treatment, coordination of care, preventive care and health maintenance to patients. The Primary Care Physician also supports clinical supervision and medical services provided by the agency.

Essential Roles & Responsibilities

Provide comprehensive medical services to patients across the life course presenting for care at UFHC.

Examine patients, using medical instruments and equipment.

Obtain complete medical history and physical data, and establish accurate diagnosis.

Perform clinical services to patients who present with illness or disease.

Order or execute various tests and diagnostic images to provide information on patient’s condition.

Review incoming reports (e.g., lab, x-ray, EKG) and follow-up in a timely manner.

Inoculate and vaccinate patients to immunize from communicable diseases.

Provide or directly oversee services and procedures appropriate in the outpatient setting.

Educate patients regarding health and illness prevention; recommend community resources.

Administer or prescribe treatments and medications, and instruct patients on proper utilization.

Refer patients to medical specialists for consultant services when necessary.

Adhere to established clinical protocols and procedures.

Maintain adequate and legible patient medical records in the established chart.

Supervise participation in and respond calmly and effectively in emergency situations.

Fulfill HRSA and health center productivity standards, such as UDS.

Follow established health center procedures for patient billing and reimbursement.

Maintain professional competence by completing CME requirements.

Participate in peer review, quality assurance, provider meetings and other clinical meetings.

Perform on‑call duties as needed including responsibilities in the electronic health record and the after‑hours telephone on‑call.

Acknowledge patient’s rights regarding confidentiality issues and follow HIPAA guidelines.

Participate in professional development activities and maintain professional affiliations and certifications.

Complete UFHC credentialing and privileged review upon hire and re‑credentialing.

Maintain professional conduct in accordance with agency policy and the Code of Ethics.

Participate in supervision meetings, staff meetings, agency‑wide meetings, utilization review meetings, in‑service training, and agency culture events.

Complete all applicable licensing requirements and ethical standards including CE hours.

Education & Experience Requirements

Graduate of an approved medical school with Board Certification.

Unencumbered New York State License to practice medicine.

Eligible to obtain and maintain a federal and state narcotic registration (DEA License).

Strong community health/public health orientation, experienced in patient care management, and demonstrates managed health care knowledge and experience.

Experience in multiple provider medical practice.

Knowledge of scheduling and medical coding, and experience with health care information systems (e.g., EClinical Works).

Strong commitment to health professions ethics including social justice, empowerment, and self‑determination.

Ability to work well with others in a team‑oriented environment, work independently, and use good judgment.

Intermediate PC skills (Excel, PowerPoint, Word, Outlook, SharePoint, Office 365).

Work Environment Blood Borne Pathogen Category (OSHA) – Category I. Job related tasks involve exposure or potential exposure to blood, body fluids, or tissue and tasks are a condition of employment.

Physical Demands

Ability to type and operate a computer with various software programs.

Remain stationary and upright during shift and move throughout the building as needed.

Position self to install equipment, including under desks.

Move equipment or supplies weighing up to 50 pounds.
