$33.65 per hour, $70,000 annually (1.0 Full‑Time Equivalent). Non‑exempt. About the Organization Mission Action has been growing rapidly, creating many new positions in different programs. Founded in 1982, Mission Action, formerly known as Dolores Street Community Services (DSCS), is a multi‑issue, multi‑strategy nonprofit organization with almost 150 employees and an annual budget of $14.5 million, based in San Francisco’s Mission District.

About the Program The Richard M. Cohen Residence is a Transitional Residential Care Facility (TRCF) in the Mission Dolores neighborhood of San Francisco. The program provides housing and supportive services to 10 individuals living with HIV/AIDS, many of whom were formerly homeless. Services include health education, case management, housing navigation, community activities, and food/meal support.

Position Summary The Care Coordinator provides care coordination, housing navigation, and therapeutic support to residents of the Richard M. Cohen Residence. The Care Coordinator supports residents with coordinating medical appointments and referrals, conducts individual sessions to meet behavioral health needs, assists in obtaining long‑term permanent housing with the Lead Health Promoter, and provides clinical guidance and training to other staff. The position reports to the Program Director and collaborates with a team of Health Promoters.

Responsibilities

Conduct a comprehensive assessment for all residents of the program and an annual re‑assessment.

Develop individualized service plans that aim to improve health and secure long‑term permanent housing; share plans with the program team and update quarterly.

Document and enter all data into client charts.

Provide appointment reminders, health education, and treatment adherence support to residents as needed, in collaboration with other program staff.

Conduct ongoing needs assessments and deliver education, case management, advocacy, or referrals to community partners as appropriate.

Identify and coordinate additional services needed, such as IHSS, financial management, or meal preparation.

Assist residents with housing navigation, including identifying appropriate long‑term housing opportunities, completing applications, obtaining documentation, and supporting the moving process.

Lead education and training sessions on psychosocial issues with staff and/or residents.

Coordinate and plan community activities for residents in collaboration with the Program Director and program team.

Participate in staff training, meetings, and development activities as requested.

Perform other duties as assigned.

Position Requirements

3+ years of experience in social work, counseling, or a similar field.

Bachelor’s degree or higher in a relevant field preferred.

Experience working with individuals living with HIV/AIDS, people with mental illness, people experiencing homelessness, and people who engage in substance use.

Ability to hold boundaries and set limits with challenging behaviors.

Bilingual (written and oral) in Spanish preferred but not required.

Experience with crisis intervention, conflict resolution, and de‑escalation.

Experience assisting clients with navigating public benefits or housing assistance preferred.

Strong written and oral communication skills.

Ability to work both independently and as part of a multidisciplinary team.

Proficiency with basic computer programs including Microsoft Word, Outlook, and Excel.

Understanding of intersectionality and the needs of communities affected by homelessness in San Francisco.

Strong commitment to housing justice and inclusive, multicultural programming.
