We are seeking an experienced senior health sector professional to oversee the successful implementation and impact of our live social investment contracts.
The Opportunity Social Finance is a non-profit that pioneered the first Social Outcomes Contract in 2010, has supported social investments across 30+ health systems and now launched Community Transformation Funds (CTF) to deliver neighbourhood health. This is a key strategic priority for Social Finance and our partners, requiring a confident and competent pair of hands to lead, guide, anticipate and resolve issues & seize impact opportunities.
You will be joining at an exciting and pivotal time with the first two CTF vanguard sites recently mobilised in West Hertfordshire and Lincolnshire. The next 12 months will be about setting these new services up for success and preparing for expansion to further sites.
Community Transformation Funds (CTF)
The first of their kind in the UK, the place-based CTFs each use ~£10M of social investment, initially from Macmillan Cancer Care, to shift care from hospitals into the community. In doing so, they support the NHS 10-Year Plan by tackling the rising demand for care and create a sustainable model that can be scaled nationally. The funds will:
Make investments into community-based preventative services to improve patient outcomes and reduce inequity;
Use outcomes-based social investment, to link payments of the service to reductions in hospital spend;
Manage the investment through a Community Interest Company (CIC), where savings from reduced hospital spending can be reinvested and future investment can be attracted;
Fund an ‘ Anchor’ programme, to co-design and test new services with VCFSE organisations to meet unmet needs and provide capability support so they can play a greater role in service delivery;
Create sustainable, long‑term improvements in the quality of life for people across each place, particularly for those experiencing disadvantage.
West Hertfordshire
In partnership with South and West Herts Health and Care Partnership. Starting with a Proactive Anticipatory Care (PAC) service providing multidisciplinary and holistic person-centred care for older people at risk of frailty with the aim of delaying the onset of health deterioration and promoting maintenance of independent living. The service will be mobilised across all four neighbourhoods in South-West Hertfordshire.
Lincolnshire
In partnership with Lincolnshire Integrated Care Board. Starting with a service to support high intensity users (HIUs) of unplanned care, providing tailored support to address wider issues impacting their physical and mental health, reduce unplanned attendance at A&E and improve and health and confidence. HIUs are often some of the most vulnerable social economic groups and more likely to be experiencing health inequalities and find planned/proactive healthcare inaccessible. The successful pilot service will be scaled across all Primary Care Networks in Lincolnshire.
Social Finance’s role
Social Finance have been contracted by each CTF CIC to provide support to the projects to enable continuous quality improvement, impact and learning, validate and claim outcomes payments, and support CIC governance and assurance.
Responsibilities Contract owner for live investments, including West Hertfordshire & Lincolnshire Community Transformation Fund CICs, and others under development.
The key elements of this include:
Interface with operational delivery teams and senior NHS management, providing strategic support, partnership working and data‑led adaptive management to shape and enhance performance and drive the achievement of financial and health outcomes and race equity.
Promote a culture of learning, continuous improvement and maximising impact potential through rigorous data‑driven analysis, reflective practice and learning communities, etc.
Ensure impact is captured and disseminated including through patient stories, testimonials, and performance & learning/evaluation reports to support influencing of national policy and practice.
Represent Social Finance on the CTF CIC Boards and Advisory Groups, and other governance groups as relevant.
Support alignment of the ‘ Anchor’ programme in each local area, in particular with regards to involvement of community partners to identify and reduce health inequalities and support refinement of propositions for future CIC investment.
Provide strategic leadership and team management for the ‘ Operational delivery’ function within Social Finance, including oversight of team resourcing, finances, team wellbeing and development.
Support the broader national CTF programme – contribute to effective governance, support collaboration with partners, pipeline development, drive equity alignment, facilitate sharing of learning, assist in transition to local ownership, and provide ongoing monitoring & evaluation
Support new site & service investment proposals, providing strategic and operational check and challenge on proposed outcome methodologies, service specifications, KPIs etc
Input to contract negotiations with new sites (and contract renegotiations with existing sites) to ensure new service contracts are robust to deliver on our impact and financial objectives.
Work with the senior leadership & operations teams to solidify and grow this area of our business, identifying further opportunities within and beyond our current work to use 10+ years of expertise in delivering transformation programmes in the health and care sector and beyond.
About You
Experience working with/for NHS and broader health/care system including the charity/community sector with an understanding of NHS organisational structures
Understanding of development, adaptive management and performance shaping of health/care services
Experience of managing complex projects with multiple stakeholders and/or pathways of care
Data analysis and financial oversight
Relationship and strategic partnership management with professionals across different disciplines and levels of seniority
Design and delivery of learning and evaluation activities
Commitment to race equity and EDI with demonstrable track record
Team management
Social investment structuring and operations of live social investments
Experience of contract management with NHS
