Social Worker (LSW) - Float, Full-Time, Days Location: 5101 Willow Springs Road, La Grange, IL 60525

Schedule Per Diem, 8a-4:30p; 2 weekend days and 4 weekdays per 6‑week schedule; One annual holiday

The Role You'll Contribute The Social Work Care Manager intervenes with patients who have complex psychosocial needs, requires assistance with eligibility determination for social programs, funding sources, community assistance, and transitions of care or discharge planning. The role offers crisis intervention, coordinates discharge planning for high‑risk patient populations, and collaborates with physicians, nurses, and the interdisciplinary team to ensure patient‑centered care through the continuum of care.

Provides grief counseling, disease adjustment support, crisis intervention, de‑escalation services, and goals‑of‑care planning.

Assesses patients and families holistically for discharge planning needs, including prior functioning, support systems, finances, and psychosocial factors.

Reviews medical records to incorporate clinical, social, and financial factors into transition‑of‑care plans.

Develops discharge plans with contingency plans throughout the hospital stay, coordinating post‑acute care and community services.

Leverages technology and follows standard processes to communicate with post‑acute services, ensuring continuity of care and accurate discharge reconciliation.

Actively participates in multidisciplinary rounds, reviewing changes in patient status and discharge plans.

Communicates with patients and families about emotional, social, and financial impacts of illness, mobilizing resources and advocating for patient empowerment.

Documents discharge planning evaluation, ongoing assessment, barriers, avoidable days, and patient/family needs.

Supports patient and family advocacy, ensuring patient rights during hospitalization.

Communicates with payors for authorization of post‑acute care as needed.

Other duties as assigned.

Qualifications

Master's degree required.

Four or more years of work experience required.

Licensed Social Worker (LSW) required.

Clinical Social Worker License (LCSW) preferred.

Accredited Case Manager (ACM) preferred.

Certified Case Manager (CCM) preferred.

Excellent interpersonal communication and negotiation skills.

Critical thinking and problem‑solving skills.

Psychosocial assessment skills.

Customer service skills.

Ability to work with diverse populations, being flexible, open‑minded, and adaptable.

Effective organizational and computer proficiency with Outlook and electronic medical records.

Knowledge of pre‑ and post‑acute venues of care and community resources.

Strong interview, assessment, leadership, and data analysis skills.

Current working knowledge of discharge planning, utilization management, performance improvement, and managed care reimbursement.

Knowledge of state and federal guidelines pertinent to Care Management.
