Overview The Age-Friendly Program Manager (AFPM) will be responsible for implementing and sustaining Age-Friendly initiatives across Maimonides Medical Center (MMC). This role will focus on governance structure development, interdisciplinary collaboration, performance tracking, and quality improvement initiatives related to the 4Ms framework (Mentation, Medication, Mobility, and What Matters). The AFPM will lead efforts to strengthen transitions of care, improve post-discharge follow-ups, support Skilled Nursing Facility (SNF) partnerships, and oversee caregiver engagement initiatives as outlined in the AIM 2.0 program.

Responsibilities

The AFPM will act as a liaison between internal stakeholders (e.g., Nursing, Geriatrics, Surgery, Emergency Medicine, Palliative Care, Medical Informatics) and external partners (e.g., SNFs, caregivers, community organizations) to enhance continuity of care and support patient-centered outcomes.

Qualifications Education & Certifications:

Master’s degree preferred in Health Administration, Public Health, Geriatrics, or a related field.

Bachelor’s degree required in Nursing, Social Work, Healthcare Administration, or related discipline.

Certification in Age-Friendly Care, Geriatrics, or Palliative Care (preferred but not required).

Experience:

Minimum 3+ years of experience in healthcare program management, quality improvement, or geriatric care initiatives.

Experience working with older adult populations, SNFs, or post-acute care settings is highly desirable.

Demonstrated expertise in data-driven decision-making, healthcare quality metrics, and project evaluation.

Skills & Abilities:

Strong interdisciplinary collaboration and stakeholder engagement skills.

Knowledge of Age-Friendly Health System principles and geriatric care best practices.

Proficiency in data reporting, outcome tracking, and quality improvement methodologies.

Excellent verbal, written, and presentation skills.

Strong analytical and problem-solving abilities to drive program effectiveness.

Pay Range

USD $125,000.00 - USD $125,000.00 /Yr.
